Central West Self Management Program The Central West Self Management Program helps people manage their chronic health conditions and improve their health. No referral required.

The program offers free, non-clinical group programs. Learn about making an action plan that supports changes that are important to you; supports for caregivers; what to do before, during and after a healthcare appointment; working with healthcare providers; and managing difficult emotions. Programs are available for individuals living with chronic conditions or at the risk of developing chronic health conditions, and their family members or caregivers.

NOW OFFERING WORKSHOPS ONLINE!



The Central West Self-Management Program has returned to full service delivery following temporary closures to support COVID initiatives. We continue to provide FREE workshops for community members throughout the region, and have pivoted to virtual service delivery. Our workshops are offered through Zoom for Healthcare.



Here are the dates for three upcoming Chronic Disease Self-Management Program workshops:


Saturday November 6, 2021 -Saturday December 11, 2021 (10:00 -12:00 pm)

Monday November 8, 2021-December 13, 2021 (6:00-8:00 pm)

Friday November 12, 2021-December 17, 2021 (10:00 am - 12:00 pm)

July 24th is International Self-Care Day – a date symbolic in that the benefits of self-care are experienced 24 hours a day, 7 days a week.

Within the past year, self-care has become increasingly important in navigating the numerous changes that were unveiled alongside the COVID-19 pandemic. We have all been so resilient in navigating these changes and discovering new, exciting activities that allow us to bring an awareness to our internal selves.

Here you will find some quick ways to incorporate self-care practices into your everyday routine. The Mental Health Commission of Canada has created a Self-Care and Resilience Guide to help you assess your current state and work towards an action plan that suits your own self-care needs. With the recent surge of COVID-19, the Central West Self Management Program will be on hold until further notice. This will enable our host organization, William Osler Health System, to redeploy vital resources. During this time we will not be able to respond to program inquiries.

The Chronic Pain Self-Management Program is a 6-week workshop developed by Stanford University that helps people who are experiencing a wide range of chronic pain conditions. Practical tools and information are shared to help develop coping skills. Participants will build confidence and motivation to manage the daily tasks of living with chronic pain. The Chronic Disease Self-Management Program is a 6-week workshop developed by Stanford University that helps people with chronic health conditions manage their symptoms and daily lives. The program provides people and their families with information and practical skills and tools to help manage their health. It provides information designed to increase confidence and motivation to manage the challenges when living with chronic health conditions. This is an educational workshop and does not replace the advice of your healthcare provider. No referral is needed.
