Supporting Unpaid Carers πŸ’š

Behind every person receiving care, there's often someone quietly doing everything they can.

Unpaid carers are the husbands, wives, sons, daughters, friends and neighbours who give their time, energy and love every single day. While incredibly rewarding, caring for a loved one can also be physically and emotionally exhausting.
That's where we can help.

Whether it's providing regular welfare visits, attending appointments, coordinating services or simply offering reassurance that someone else is keeping an eye on things, we can help lighten the load.

Our role isn't to replace family – it's to support them.
Because carers need caring for too. πŸ’œ

Working with Solicitors 🀝

Many local solicitors already use our Care Coordination and Case Management service and some have even trusted us to oversee the wellbeing of their own relatives. We believe that's one of the strongest testimonials we could ever receive.

When a solicitor has concerns about a vulnerable client, our role is to provide the practical, ongoing support that sits alongside the legal work. Whether a client is living at home or in a care home, we ensure their wellbeing remains the priority.

Our Care Coordinators take on many of the responsibilities that often fall to solicitors or family members who simply can't be there when they're needed. This helps avoid delays during critical moments and provides reassurance that someone is always looking out for the client's best interests.

With over 100 years of combined experience in health and social care, our team offers far more than care system navigation. In some circumstances, we can even act as Health and Welfare Attorney where appropriate.

And because life's challenges don't only happen between 9 and 5, neither do we.

Working with Health & Social Care Professionals

The best outcomes happen when people work together.

At My Care Coordinator, we work alongside health and social care professionals to provide independent, person-centred support that complements existing services. Our team works closely with care homes, home care agencies, hospital and community social work teams, Community Mental Health Teams and the CQC.

Whether supporting someone living at home or in a care home, we help coordinate services, advocate for individuals and provide reassurance to families, especially when they can't always be there themselves.

Our service is trusted not only by private families but also by solicitors, the NHS and local authorities through personal budgets. A memory health lead had one of their clients call us a godsend recently.

If you're a health or social care professional or know a family who could benefit from additional support, we'd love to hear from you. 01476 851620

Working with Families πŸ‘¨β€πŸ‘©β€πŸ‘§β€πŸ‘¦

When someone you love needs more support, it can quickly become overwhelming.

You may be trying to juggle work, family life and your own responsibilities, while worrying about whether they're safe, happy and receiving the right care. If you don't live nearby, that worry can feel even greater.

That's where My Care Coordinator can help.

We work alongside families, becoming a trusted point of contact and helping to take the pressure off. Whether it's coordinating with health and social care professionals, attending appointments, managing day-to-day issues or simply being there when something unexpected happens, we're here to provide reassurance and practical support.

With over 100 years of combined experience across our Care Coordination team, we understand the challenges families face and are committed to helping your loved one live as safely and independently as possible.

Because sometimes the greatest support we can offer isn't just for the person receiving care... it's for the whole family.

If you're worried about a loved one and don't know where to turn, we'd be happy to have a conversation.

The best partnerships are built on trust. 🀝

One of the things I enjoy most about meeting professionals in the work we do, is those who genuinely care about helping people.

A recent conversation with Gareth Day from Lloyd Legal Services reminded me that no one business can be an expert in everything, but by working together we can offer clients the support they really need.

For those helping clients with later life planning, getting their affairs in order or navigating care decisions, having access to trusted legal expertise can make all the difference.

Likewise, if you're a legal professional whose clients could benefit from independent care coordination, advocacy or practical support, I'd love to explore how we can work together.

Strong referral partnerships aren't about passing business around. They're about ensuring clients receive the right advice, at the right time, from the right people.

A Day in the Life of a My Care Coordinator 🀝

People often ask what our Care Coordinators do all day. The simple answer is whatever is needed to help our clients live safely, independently and with less stress and often no two days are the same:

πŸ’š Resolving a client's BT phone issue so they stay connected with family and essential services.
πŸ’š Supporting a Social Worker during a capacity assessment.
πŸ’š Helping manage finances and preventing missed payments or unnecessary bank charges.
πŸ’š Coordinating care for someone living with complex needs.
πŸ’š Arranging equipment collections following a move into a care home.
πŸ’š Organising medication orders and Meals on Wheels.
πŸ’š Redirecting post, completing Attendance Allowance applications and handling important paperwork.
πŸ’š Liaising with families, healthcare professionals and care providers to keep everything running smoothly.

Often, lunch is a packed lunch πŸ₯ͺ eaten between visits while catching up on phone calls and emails.

Every task, no matter how big or small, helps reduce stress for our clients and gives families peace of mind.
If you think we could support you, a loved one, or work alongside you professionally, we'd love to hear from you

We allocated an experienced Care Coordinator who is available 7 days a week, 12 hours a day to visit, monitor, advocate and address any care issues or practical problems that go wrong in the home. We are the eyes and ears for the family abroad.

Moving into a care home is often just the beginning of a much bigger transition.

While families are focused on ensuring their loved one settles into their new home or sometimes there is no family to help, there is often a long list of practical tasks left behind that can feel overwhelming.

At My Care Coordinator, we can help with those important jobs, including:

πŸ’š Returning or collecting equipment
πŸ’œ Arranging decluttering and house clearances
πŸ’š Preparing a property for sale or rental
πŸ’œ Organising utility bills and household accounts
πŸ’š Redirecting post and updating important records
πŸ’œ Liaising with estate agents, solicitors and other professionals

Having supported many families through this process, we understand that every situation is different. Our role is to take away some of the stress, provide practical support, and ensure everything is handled with care and dignity.

Sometimes it's the little things that make the biggest difference during what can be an emotional and challenging time.
