National Programme of Talks On "It's a Matter of Heart"

Medical researchers tracking India’s cardiovascular disease (CVD) burden, are witnessing cases more frequently compared to the past two decades, and it’s a unanimous conclusion — heart diseases cause the maximum deaths in India and the number is on a steady rise. Since CVD strikes Indians nearly a decade earlier than other populations, a large number of heart attacks happen to people between 30 to 69 years old. Despite wide variations in the prevalence of risk factors, CVD has emerged as the leading cause of death in all parts of India, including poorer states and rural areas, mainly due to lifestyle changes. This programme shall probe the reasons behind the prevalence of cardiovascular disease and highlight preventive and palliative measures.

In this video, Dr. O.P. Yadava, Chief Executive Officer & Chief Cardiac Surgeon of National Heart Institute, New Delhi, talks about Total Arterial Bypass Surgery.

Whenever a Bypass Surgery is done, only arterial conduits are used which is then known as total arterial myocardial revascularization. It can be performed using various configurations. Free grafts, in situ grafts, single grafts, or sequential grafts are used in bypass surgery. In sequential grafts, one conduit can be used to bypass two arteries. Dr. Yadava will also describe that a free graft can be attached to a pedicle graft and can-do T or Y configuration of total arterial revascularization known as T or Y grafts.

BYPASS SURGERY IN LOW EJECTION FRACTION?

When we talk about bypass surgery in low ejection fraction. The results of the bypass surgery are not very good if the heart is damaged. Heart failure with reduced ejection fraction happens when the muscle of the left ventricle is not pumping as well as normal. The ejection fraction is 40% or less. The amount of blood being pumped out of the heart is less than the body needs.

At what ejection fraction is heart failure?
Normal EF is in the range of 55% to 70%. As the percentage falls, heart failure is getting worse. In general, if the EF falls below 30%, it's relatively severe. A reading of 20% or below is very severe heart failure.

Surgeons perform thousands of heart bypass surgeries each year and many of those who have the surgery get relief from their symptoms without needing long-term medication. The more severe the heart disease, the higher the risk of complications.

The results of total arterial bypass surgery are mostly superior to venous revascularization. Almost 10 – 15 % of the venous grafts are blocked by the end of one year. Whereas the patency rates of arterial grafts are high even at 10 – 20 years. Total arterial bypass surgery revascularization is done when a patient is young but it also depends upon various factors.
