The video demonstrates tackless mesh fixation using 2-0 Prolene, with emphasis on:
• Complete coverage of the myopectineal or***ce
• Ensuring the mesh lies completely flat without folding or rolling
• Appropriate mesh overlap and positioning
• Safe, selective suture fixation
• Rechecking mesh position after returning the peritoneal flap
• Ensuring there is no crumpling or displacement of the mesh after peritoneal closure
The goal is simple: complete MPO coverage, a flat and stable mesh, and a smooth peritoneal closure.

When unexpected bleeding occurs during laparoscopic surgery, the first priority is not speed—it is control.

Bleeding can be one of the most stressful situations in minimally invasive surgery. Remaining calm, maintaining orientation, and following sound surgical principles are often more important than any instrument in your hand.

In this operative video, I demonstrate the management of an unexpected venous bleed during laparoscopic right hemicolectomy, highlighting a systematic approach to bleeding control while avoiding blind clipping or indiscriminate use of energy.

Key principles discussed:
• Stay calm and maintain orientation
• Temporary compression to control bleeding
• Suction to improve visualization and identify the exact source
• Avoid blind clipping or electrocautery
• Secure haemostasis under direct vision
• Reassess before proceeding

Laparoscopic ICG-Guided Heller's Cardiomyotomy

Achalasia cardia is a challenging esophageal motility disorder where complete division of the lower esophageal sphincter is essential for durable relief of dysphagia, while preserving the integrity of the underlying mucosa.

In this video, I demonstrate a Laparoscopic ICG-Guided Heller's Cardiomyotomy for Type II Achalasia with Sigmoid Esophagus. By using intraluminal Indocyanine Green (ICG) fluorescence, the esophageal mucosa is visualized in real time, allowing precise identification of residual muscle fibres and facilitating a safer and more complete myotomy.
