TAPP Masterclass Series | Mesh Deployment, Placement & Fixation
Sharing the next video in my TAPP Masterclass Series, focusing on the important final steps of mesh deployment, positioning and fixation during laparoscopic TAPP inguinal hernia repair.
The video demonstrates tackless mesh fixation using 2-0 Prolene, with emphasis on:
β€’ Complete coverage of the myopectineal or***ce
β€’ Ensuring the mesh lies completely flat without folding or rolling
β€’ Appropriate mesh overlap and positioning
β€’ Safe, selective suture fixation
β€’ Rechecking mesh position after returning the peritoneal flap
β€’ Ensuring there is no crumpling or displacement of the mesh after peritoneal closure
The goal is simple: complete MPO coverage, a flat and stable mesh, and a smooth peritoneal closure.

Laparoscopic TAPP Pseudosac Dissection | Inguinal Hernia Repair

Sharing the next video in my TAPP Masterclass Series, focusing specifically on pseudosac dissection during laparoscopic TAPP repair of a direct inguinal hernia.

Pseudosac dissection is an important technical step in TAPP. Identifying the correct preperitoneal plane and carefully reducing the pseudosac allows complete exposure of the myopectineal or***ce and facilitates optimal mesh placement.

The video demonstrates the surgical anatomy, step-by-step dissection and technical principles for safely tackling the pseudosac.

Laparoscopic TAPP Inguinal Hernia Repair | Step-by-Step

I am pleased to share the first video in my TAPP Masterclass Series on Laparoscopic TAPP Inguinal Hernia Repair.

The video demonstrates the procedure step-by-step, with emphasis on preperitoneal dissection, identification of key anatomy, complete exposure of the myopectineal or***ce, and mesh deployment.

This series is aimed at General Surgeons, GI Surgeons, laparoscopic surgeons, residents, and surgical trainees, with a focus on practical operative technique and surgical anatomy.

When unexpected bleeding occurs during laparoscopic surgery, the first priority is not speedβ€”it is control.

Bleeding can be one of the most stressful situations in minimally invasive surgery. Remaining calm, maintaining orientation, and following sound surgical principles are often more important than any instrument in your hand.

In this operative video, I demonstrate the management of an unexpected venous bleed during laparoscopic right hemicolectomy, highlighting a systematic approach to bleeding control while avoiding blind clipping or indiscriminate use of energy.

Key principles discussed:
β€’ Stay calm and maintain orientation
β€’ Temporary compression to control bleeding
β€’ Suction to improve visualization and identify the exact source
β€’ Avoid blind clipping or electrocautery
β€’ Secure haemostasis under direct vision
β€’ Reassess before proceeding

This video is intended for surgeons and trainees interested in advanced laparoscopic surgery and the practical management of intraoperative challenges.

Laparoscopic ICG-Guided Heller's Cardiomyotomy

Achalasia cardia is a challenging esophageal motility disorder where complete division of the lower esophageal sphincter is essential for durable relief of dysphagia, while preserving the integrity of the underlying mucosa.

In this video, I demonstrate a Laparoscopic ICG-Guided Heller's Cardiomyotomy for Type II Achalasia with Sigmoid Esophagus. By using intraluminal Indocyanine Green (ICG) fluorescence, the esophageal mucosa is visualized in real time, allowing precise identification of residual muscle fibres and facilitating a safer and more complete myotomy.

The video includes:
β€’ Clinical evaluation and operative planning
β€’ Preoperative intraluminal ICG preparation
β€’ Step-by-step laparoscopic Heller's cardiomyotomy
β€’ Fluorescence-guided esophageal and gastric myotomy
β€’ Technical pearls for safe dissection
β€’ Intraoperative endoscopy and Dor fundoplication

I hope this operative video serves as a useful educational resource for surgeons, trainees, and everyone interested in advanced minimally invasive GI surgery.
