- photocopy ti valid Id ni representative

Part I:

What: Re-orientaion, Registration and Organization of PWDs
When: February 12, 2026 @ 8AM
Where: Municipal Gymnasium
Who: Persons with Disability (registered and not yet registered), only those who are interested

What to bring:
1. For new applicants- Medical Certificate (latest with name,signature and license # of the attending physician), photocopy of Birth Certifcate/PSA, and 1x1 ID Picture
2. For old with expired or soon to expire (within this year) PWD ID- Bring the Old PWD ID, 1x1 ID picture and Medical Certificate (latest with name,signature and license # of the attending physician)
3. For damaged ID- Just bring the damaged PWD ID,1 x1 ID picture and Medical Certificate (latest with name, signature and license # of the attending physician)

Note:
1. Nu adda masukatan iti spelling ti nagan, birthdate ken s*x (male and female) pakitugot latta apo photocopy of Birth Certificate/PSA
2. Mabalin representative kadagiti unable nga PWDs tayo basta itugot dagiti requirements

Who: Solo Parents (Only those who are interested and willing to register)

List of Requirements to bring: (Affidavit forms will be provided by the office)
***For new and old applicants:
A. Bring photocopy of Birth Certificate/s of your child/children 22yrs old and below or 22yrs old above but with disability and unable to perform daily task/work
B. For widow/widower- bring photocopy of death certificate of spouse
