โœ”๏ธDocumentary Requirements for Beneficiaries claiming Personally, Each qualified beneficiary must submit the following:
๐Ÿ’™4 original copies of the school ID or any valid government ID with three (3) original sign
๐Ÿ’™4 original copies of Barangay Certificate of Indigency.

โœ”๏ธFor Beneficiaries claiming through an Authorize representative:
๐Ÿ’™4 original copies of prescribed authorization letter, duly signed by the Beneficiary ,the Authorized Representative, and the Punong Barangay
๐Ÿ’™4 photocopies of valid ID of the Authorized Representative, each bearing the representative's signature;
๐Ÿ’™4 photocopies of the valid ID of the Beneficiary ,each bearing beneficiary's signature

Applicants with previously lacking or incomplete documentary requirements may submit their documents at the payout venue on the scheduled date.

2. Studyante han Senior High School ngan mayda grado nga dire maubos hit 80%;
3. Studyante ha Kolehiyo ngan enrolled hin bisan 12 units han naglabay nga semestre ngan may grado nga dire maubos hit 3.0;
4. Waray scholarship tikang ha National or Provincial nga ahensya;
5. Dire benepisyaryo hit 4Ps.

Requirements:

1. Application Form
2. Registration Form han naglabay nga semestre para han Kolehiyo;
3. Voter's Certification hiton guardian (kon nakikiukoy la hit guardian)
4. Certificate of Grade for College/ Form 138A or Report Card for Senior High

Instruction:

1. Duoka an iyo Barangay Kagawad Committee Chair on Education ngan aro hin kopya han Application Form ngan han lista han mga Requirements; ngan
