The applicant must be a bonafide resident of Maluso Municipality. He must secure Barangay Certification of his/her residency, Municipal Engineerโs Permit, 2 pcs. Valid ID (to be photocopied), Community Tax Certificate (Cedula), 2 pieces 2x2 & 1 pc. 1x1 colored ID pictures (taken not later than 6 months), fill up the Concessionaireโs Information Sheet Form and to be submitted to the Customer Servic
es Desk. Upon completion of the requirements, inspection and investigation of location by the maintenance team will follow. Map location will also be prepared. The applicant should undergo policy orientation of the district. Signing of Contract of Agreement will follow. After payment of registration and other charges, the contract will be forwarded to the General Manager for approval. For installation.
2. RE-CONNECTION / RE-OPEN
a. Sign the service request form. After payment of the corresponding charges, the consumer should undergo a pre-orientation seminar of MAWADs Policy. In case the disconnection took place three (3) months ago, the concessionaire is to fill up the Concessionaireโs Information Sheet Form. Approval of the General Manager.
3. TRANSFER OF SERVICE CONNECTION
a. A request for transfer of service connection should be made to the Customer Services Desk at the Commercial Section. Maintenance team will inspect and investigate the area for transfer. A sketch map will also be prepared by the team. If the area is accessible for transfer the Engineering Assistant will prepare the Job Order for approval of General Manager after payment of transfer fee and other charges.
4. TRANSFER OF OWNERSHIP
a. A deed of sale of water service connection or the likes should be presented to the Customer Services Desk. Fill up the Concessionaireโs Information Sheet Form, CTC (Cedula), Brgy. Clearance, 2 pcs. 2x2 ID pictures, 1 pc. 1x1 ID picture and 2 pcs. Valid ID (to be photocopied) for preparation of new Contract of Agreement.
