𝗤𝘂𝗮𝗹𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻:
1. Must be a resident of Rosario, La Union.
2. Must not have an existing scholarship grant from other government agency (National and Local)
3. Maintain a weighted average of at least 85-percent with no grade lower than 80 in any of the academic/learning course.

1. Form IA (MEAP Application Form)
2. Report Card / Class Card/ Certification of Grades SY 2024-2025
3. Certificate of Enrollment SY 2025-2026
4. Endorsement from Punong Barangay
5. Certificate of Indigency from their Barangay
6. Affidavit of Non-Filing of Income Tax Return
7. Letter of intent addressed to the Honorable Mayor
8. Photocopy of Birth Certificate
9. Picture of the student planting (5) fruit-bearing seedlings. (Printed copy).

The MSWD Staff distributed Relief Assistance (Family Food Packs, Sleeping Kits, Hygiene Kits) to the fire victim last May 18, 2025, at Barangay Tanglag, Rosario, Rosario, La Union. Assessment was also done by the MSWD Staff and instructed them to provide the requirements for their financial assistance.
