REQUIREMENTS:
๐ชช PRIMARY DOCUMENTS:
โข PSA-issued Certificate of Live Birth and one (1) government-issued identification document which bears a full name, front-facing photograph, and signature or thumb mark
โข Philippine Passport or ePassport issued by the Department of Foreign Affairs (DFA)
โข Unified Multi-purpose ldentification (UMID) Card issued by the Government Service lnsurance System (GSIS) or Social Security Systen (SSS); or
โข Studentโs License Permit or Non- Professional/Professional Driverโs License issued by the LTO.
(In case there is/are discrepancy/ies in the entry/ies in PSA-issued Certificate of Live Birth and the government-issued ID presented, the entry/ies in the PSA-issued Certificate of Live Birth shall prevail.)

Ang National ID program ay programa ng Phil. Statistics Authority (PSA) at PhilSys, sa pakikipagtulungan ng City Civil Registry ng Taguig City.
๐ชช NAKAPAG-APPLY NA AKO NOON PERO WALA PA RIN AKONG NATATANGGAP NA NATIONAL ID. PUWEDE BA AKONG MAG-APPLY ULIT?
One time lang po ang registration sa National ID. Hindi na puwedeng umulit.
