A heartfelt thank you to the members of NACDD's ADRD Community-Clinical Linkages Expert Panel for sharing your time, expertise, creativity, and candid feedback over the past year. Through a series of human-centered design sessions, this remarkable group of leaders from public health, health care, aging services, community-based organizations, and lived experience helped us tackle a challenging question:
How can public health help strengthen community-clinical linkages that support brain health, risk reduction, early detection, care planning, and ongoing support for people affected by Alzheimer's disease and related dementias?
The result is the ADRD Community-Clinical Linkages Public Health Implementation Framework, a practical tool designed to help public health agencies strengthen partnerships, build infrastructure, and advance coordinated brain health pathways across communities and care settings. The framework will be piloted with CDC-funded Building our Largest Dementia (BOLD) Infrastructure recipients, and then we will share it more broadly.
