Dear Surviving Family Member,

The Department of Defense is committed to providing an enduring remembrance of service members who died while serving our nation. In that spirit, on Veterans Day, we are launching the Military In Lasting Tribute memorial, an online memorial honoring service members who died while on active duty from 1985 to the present. It will be the only DOD memorial to include peacetime deaths.

Once the memorial launches, family and friends of eligible service members will be able to submit their loved ones through a form on the website to be honored on the memorial. However, we are providing you with an opportunity to submit your deceased service member’s name for inclusion on the memorial in advance of the website launch on Nov. 11, 2021.

If you are interested in this opportunity to have your service member on the memorial at launch, we ask that you provide by email information from your service member’s Report of Casualty DD Form 1300, a photo of your service member, if available, and some information about yourself. Your submission should include:

• Your first and last name, relationship to your service member, phone number and email address.
• Your service member’s information from the DD Form 1300 (be sure you provide the information listed on the form so we can match it against DOD records):
o First name
o Last name
o Middle name
o Service branch
o Component
o Rank
o Date of death

If you provide a photo, it must:

• Show only your service member, dressed in their military uniform or business attire. We recommend the photo be in portrait orientation.
• Be in PNG or JPG format, with a maximum file size of 5MB.

The photo must have either been taken by the submitter, or the photographer must have granted permission for its use.
If you don’t have a photo that meets these requirements, you can opt to show your service member’s service branch seal instead.

Email the information above and your service member’s photo to [email protected] by Oct. 20, 2021.

In the hearts of our nation, no one holds more honor and respect than our deceased service members. Our nation and our military are eternally grateful to them — and to you, their survivors. Help us pay tribute to these heroes by submitting their names to be honored on the memorial.

A cherished friend Jamie Westbay has stood by our side with her full support to us not just after Jesse's death, but each day, month and year since. She has been my source of hope and I am certain a real live angel on this earth. My thoughts had migrated towards this being the first year Jesse would not have a wreath, and she had no idea I felt this way! One thing about true friends, they know your heart without speaking a word. To our surprise she purchased and placed a wreath AND flag to remember Jesse this year and our hearts are now full💔❤️
CASCOM's Foreign Liaison Officers: LTC Stuart Vernon (UK), LTC Peter Stubakow (Germany), LTC Luc Vernaz (France) and Major Ulysses Pereira Braga (Brazil) participated in the 'Run for the Fallen' this weekend. The LNOs ran a route that was lined with photographs of fallen Soldiers here at Fort Lee. CASCOM LNOs showed support for this great initiative in remembering the heroes that have fallen in the USA and in all our nations.
