You can schedule up to 7 pm 2 days before the clinic is held and Monday or Tuesday clinics until 7 pm the previous Friday.

ODP is holding vaccine clinics later this week for they people they support and their families. Call your Supports Coordinator for the details.

If they don't know about it, then ask for the supervisor! These clinics are specifically set up for us and NOT open to the public.

Presenter: Michelle Gwinn Nutter, a Pennsylvania-certified teacher and former Safe and Supportive Schools Manager for the PA Center for Safe Schools, is an Education and Outreach Program Manager and Civil Rights Outreach Specialist. Michelle provides training and technical assistance for schools and community organizations. She is a nationally recognized speaker and frequently serves as a facilitator for the Student Problem Identification and Resolution of Issues Together (SPIRIT) Program coordinated by the U.S. Department of Justice’s Community Relations Service. She is a certified Olweus Bullying Prevention Program trainer and a certified Partners Against Hate trainer. She assists schools in the prevention of and effective response to bullying, cyberbullying and bias-related tension incidents. Michelle received her B.A. Degree in English Education from Messiah College and a M.S. in Education Law.
Michelle participated in the 2016 White House Bullying Prevention Summit and the 2016 Federal Bullying Prevention Summit.

For any Common Law Employers who have received a communication asking you to fill out a PA-100 and alerting you that you owe withholding...

"Palco is working closely with the department of labor to correct this issue. There were small SUTA withholdings from the SSP’s paycheck during 2020 that should not have happened as the CLE’s did not meet payroll thresholds to qualify for SUTA during those quarters. This caused the Department of Labor to request a PA100 to be filed. Palco is working to amend the SUTA accounts, refund the money to the SSP that was withheld, and close the SUTA account for the relating quarters with the Department of Labor. The CLE has nothing to worry about, we have been working with a rep at the DOL for a couple weeks now in correcting these issue. All financial penalties that are incurred are Palco’s responsibility to pay as the Employer Agent."

FEMA Announces Center City Philadelphia Vaccination Center Operating Details | FEMA.gov

Some accessibility information about the FEMA vaccination site in Philadelphia.

This is an appointment-only site for Philadelphia residents in Phases 1A and 1B. Those interested can use the vaccine interest form or call 311 to register.

The site will have signage with directions for clients with access and functional needs and non-English speaking clients in five languages, with additional language support on call as needed. There will be an assisted check-in area and process for clients with access and functional needs. There will be on-site translation in Spanish and American Sign Language (ASL) with additional language capability on call as needed.

Wheelchairs will be available.

While Philadelphia residents are able to complete the vaccine interest form online in English and Spanish at: https://covid-vaccine-interest.phila.gov/ we know many families do not have the technology to access this form or important information around COVID-19 . There is a phone number and email where questions can be answered. Residents can additionally get assistance with completing the interest form and translation can be provided for non-English Speaking residents.

Please share that Philadelphia Residents can call 215-685-5488 or email: [email protected] for these supports. Specialists are available from 8:30am-6:00pm Monday-Friday and 9:00 am-5:00pm Saturday and Sunday. Thank you.

This link will give you a letter to show as proof you're a caregiver when getting your vaccine.

Scroll all the way to the bottom of the page, there is a little box you can check to attest that you are an unpaid caregiver. When you check that box, a link will appear to a letter that you can use to provide any vaccination site that requires some kind of proof of eligibility to receive the vaccine during Phase 1A as a family member/unpaid caregiver.
