PACE (Program of All-inclusive Care for the Elderly) is a comprehensive health care program for seniors who have disabilities or health concerns and wish to remain in their ho
me while receiving all needed medical and supportive services. PACE offers an entire continuum of care from medical services to medications, from nutritious meals to transportation – all personalized to meet your specific needs. This model of health care has been in existence for over 30 years throughout the United States. The program focus is on prevention, wellness, and
a commitment to enhance the quality of life of seniors and their families through care and support. Eligibility
Carolina SeniorCare serves residents of Davidson, Rowan, Davie and Iredell Counties who are certified by the State of NC as eligible for nursing home care. You must be 55 or older and able to live safely in the community at the time of your enrollment. Costs
PACE is typically funded by Medicare and Medicaid to deliver necessary care. Our services may be provided at no additional cost if you are covered by
Medicare and/or Medicaid. If you qualify only for Medicare, there is a set monthly fee you will be asked to pay. Private pay seniors who aren’t covered
by Medicare or Medicaid, but choose to pay for our services, may also be accepted. Upon enrollment in the program, the participant agrees to receive all
health care services through PACE’s health care network of providers. Participants may be liable for the cost of medical services from an out-of-network
provider or without prior authorization with the exception of emergency services. PACE is a voluntary enrollment program; a participant may disenroll at any time. Our Care Team
Carolina SeniorCare PACE has doctors, nurses, therapists, dieticians, pharmacists, social workers, personal care aides and activities staff who work as
a team to help you stay at home and in the community. You’ll have peace of mind knowing a safety net of services is available to take care of your
needs. PACE Day Center
Our day center is open five days a week, with transportation services, and provides a fun and friendly place where you will find medical, rehab
and support services. You can take pleasure in meals with friends, enjoy social activities and get the support you need to make life more fulfilling. In-Home Services
We provide a variety of services in your home from vital home health care, to personal care, to help with homemaker services and routine chores. We Deliver All Inclusive care
Carolina SeniorCare PACE is a not-for profit provider of services under the sponsorship of United Church Homes and Services (UCHS), a ministry offering
senior services and senior housing for more than 40 years. Carolina SeniorCare
