Property Amenities:
Pathway House is a privately-owned, 50-bed transitional care facility for chemicallydependent adults. Accommodations in the three-story homes include single, double and
multiple occupancy rooms; dining, meeting and recreational rooms; administrative
offices; and a private yard. Pathway House provides drug-free, supported living for adults,
while maintaining individual dignity and self-esteem. Recovery depends largely upon the
initiative of each resident. As a result, Pathway House fosters personal growth
throughout the recovery process.
Qualifications:
Adult men and women age 18 and older with chemical dependency, substance abuse and
mental illness problems. Pathway House accepts most offenders, with the exception of
level 3 s*x offenders.
Other Information:
• Minimum stay is 60 days
• Residents must attend weekly individual and group counseling sessions and threeAA
meetings per week
• Program services help residents develop independent living skills, obtain employment
or education, make constructive use of leisure time, improve interpersonal
relationships, and take on new levels of responsibility

Length of program: duration of parole and/or completion of Intensive Supervised
Release.
Our program accepts male offenders, except for those who have been accused of
murder. Those who violate parole while in the program are not guaranteed return but
will be evaluated on a case by case basis. We also accept Level 1, 2 and 3 s*x offenders.
Eligibility Requirements:
• Participation in Medical Assistance or Health Insurance. Establish care with local
medical provider.
• Evaluation for enrollment into Adult Rehabilitative Mental Health Services (ARMHS),
and if eligible, enrolled and 100% compliant throughout the program. A participant
choosing to withdraw from ARMHS may constitute removal from our program.
• Abstinence from all mind-altering chemicals except for ni****ne, caffeine, and those
prescribed and monitored by a medical provider.
• The participant will pay $500 per month for a room and utilities.
Details of program/facilities:
• The house is a 4-bedroom home. The participant will occupy one of three bedrooms
with key lock and share a full bathroom with 2 other occupants. Common areas
include kitchen, living room, dining room, additional ¼ bath, and sauna.
• Parole Officer will have keys to the house and bedroom of those they have authority
over.
• No alcohol is allowed in common areas. No illegal drugs are allowed on premises.
Narcotics prescribed by provider need to be locked in a safe obtained and paid for by
tenants and kept in their room. A tenant who is not participating in the program may
keep alcohol in the privacy of their own room. A participant in the program may not
possess alcohol.
• Participant will establish a budget and keep to it. Participant will begin working
towards employment. Participant will be a good neighbor.

Shelter for 34 men and 6 women for a 30 days up to 90 days stay. Hours of shelter use is
from 5:30 p.m. to 8:30 a.m.
Men and women seeking shelter must go to the Adult Shelter Connect which is located at
215 South 8th Street, Minneapolis, MN.
Other Information:
Shelter Accommodations include:
• Ability to work on goals/needs with a Case Manager.
• Light Breakfast, Lunch Packs, and Dinner (at 7pm) are available.
• A safe living environment that offers dormitory-style bedrooms.
• Showers and laundry facilities.
• Access to secure personal storage space.
• May use the address for personal mail service.
• We will work with offenders, s*x offenders must register.
Permanent Supportive Housing
Property Information:
Our Saviour’s Permanent Supportive Housing Program serves long-term homeless men
and women who live with a mental illness, physical disability, and/or substance use
disorder. For as long as needed, we support more than 100 residents in affordable rental
housing around Hennepin County. After years (even decades) of homelessness, residents
have truly found their way home. With ongoing support, they also work to improve their
overall health and wellness.

East Haven’s two primary focus areas are sobriety and communication. East Haven offers
apartment style living where 3 residents share a 2-bedroom apartment and are required
to cook, clean, shop, and do laundry on their own. East Haven also offers groups on
Relapse Prevention, Music Group, Life Skills, Anger Management, Financial Fitness, HIV
Education, and other varying groups.
Other information:
The Length of stay varies between 60-120 days, and the program is for men, 18 years or
older. East Haven accepts offenders, with the exception of level 2 and 3 s*x offenders.
However, East Haven will take level 1 s*x offenders on a case by case basis.
Qualifications:
• Rule 25 or MN state insurance funded
• Must have recently completed a high-intensity residential treatment program
successfully
• Have the ability to function independently and develop independentstructure
• We expect clients to be able to develop said structure by volunteering, finding
employment, or going to school for a minimum of 20 hours per week

LifeNet is a mental health clinic, which also provides chemical dependency counseling, supportive employment and a range of permanent supportive housing options. All of the housing units are permanent supportive (fully furnished) housing, with no requirement that the
client move out at any specified time. These units are single room occupies and shelter plus units. There is some Section 8 Housing that is connected through The Bridge, which may be fully furnished if outside assistance is secured. Families with children are welcome, as well as
individuals. At least twice a month, the client is required to meet with their case manager as well as attend at least 1 group session. All units are substance free and are fully furnished. Case managers track doctor's appointments, meetings with the case managers and medication compliance. A monthly drug screening is conducted and the cleanliness of the home is inspected. Clients pay 30% of their income, or nothing if they have no income. If the clients are not working, LifeNet has an incentive to work program.
Requirements:
• Homeless (as defined by HUD) – facing eviction within 2 weeks without a suitable destination for human habitation.
• Suffering from a severe mental disability, such as major depressive disorder, bipolar disorder, and/or paranoid schizophrenia
• *LifeNet has housing opportunities for all criminal backgrounds including s*x offenders,
arsonists, and violent offenders. Please keep in mind that the more problematic the criminal history, the less options available.
