Brief imaging findings:
Longitudinal grayscale ultrasound images of the right Achilles revealtrbrew an irregular hypoechoic region in the Achilles tendon approximately 6 cm proximal to the calcaneal insertion, extending the width of the tendon (superficial to deep).

Bonus: what is the physical/sonographic provocative test to assess this lesion?
- Thompson test: with the patient prone and the foot over the edge of the table, squeezing of the calf should elicit planter flexion of the foot if the tendon is intact.
- Was positive/abnormal in this patient.

Linear echogenic foci within the renal collecting system (blue arrows) that demonstrate dirty posterior shadowing consistent with gas. Contrast-enhanced CT was performed for confirmation of the location of the gas (blue arrow), which was confirmed to be contained to the collecting system.

Diabetic patients are susceptible to necrotizing infections such as emphysematous pyelitis (our case) and emphysematous pyelonephritis (surgical emergency).
