So… what is Enhanced Care Management (ECM)? πŸ’­

It’s real, hands-on support for people navigating complex challenges like:
β€’ Homelessness
β€’ Serious mental illness
β€’ Frequent hospital visits
β€’ Transitions like reentry from incarceration

Through ECM, each person is connected to a Lead Care Manager who helps coordinate physical health, mental health, and social services.

At TPCP, that means:

πŸ’š Supporting long-term recovery

Enhanced Care Management (ECM) is helping individuals with complex needs navigate healthcare, housing, and support services all in one place, with one dedicated care team.

Since 2022, over 451,000 Medi-Cal members have been connected to person-centered care coordination.

At TPCP, this is exactly the work we show up for every day. Walking alongside individuals, meeting them where they are, and helping them access the care and support they deserve. 🀝
