UPAC Community Health Workers Communication & Outreach Services We provide culturally and linguistically appropriate COVID-19 information and resources to Native Hawaiian Pacific Islander & Asian populations.

CHWCOS is funded by the County of San Diego in support of the Live Well San Diego vision. UPAC's Community Health Workers can provide:

-- Vaccine Assistance: Our staff can assist with navigating San Diego county COVID-19 resources. We also offer in-person accompaniment at COVID-19 testing and vaccination appointments to minimize barriers such as language and transportation.

--COVID-19 Education

& Outreach: Our Community Health Workers are available to speak to groups in person, table at public events and host online education sessions.

--Printed Materials: We can provide multilingual printed materials from the County of San Diego, CA state and CDC including COVID-19 info and community resources.
