HCAI is proud to introduce the California Rural Health Transformation (CalRHT) program.

CalRHT is California's approach to the federal Rural Health Transformation Program (RHTP), backed by a $233.6 million award from the Centers for Medicare & Medicaid Services (CMS) for Federal Fiscal Year 2026. Led by the Department of Health Care Access and Information (HCAI), CalRHT is designed to strengthen health care delivery in rural communities where access challenges, workforce shortages, and infrastructure gaps create unique barriers to care.

The program is built around three core initiatives:

🏥 Transformative Care Model – strengthening primary, maternity, & specialty care through Rural Collaboratives

👩‍⚕️ Workforce Development – recruiting and retaining health professionals in rural areas

💻 Technology & Tools –support providers in a variety of needs including modernizing electronic health records, practice management, screening tools, population health systems, telehealth and eConsult platforms, optimizing interoperability, and improving revenue cycle management

Together, these efforts support a shared vision: a connected and resilient rural health system, helping ensure rural Californians can access timely, person-centered care closer to home.
