Mission: The Robins AFB School Liaison Program is committed to outreach, advocacy, and partnership initiatives that generate real-time solutions for military-connected students in grades PreK - 12 as they face global educational challenges and transitions.

📣Attention, Parents!

Check out the information below regarding school and sports physicals. The patient must be assigned to a 78th Medical Group primary care manager to receive either physical.

🏫 School physical (Call 478-327-7850 to schedule):
1. Complete GA form 3300 and bring a copy to the appointment: https://georgia.gov/get-required-health-records-attend-school -29616-step-3
2. Complete GA form 3231. Request the form via Patient Portal or complete in person at the immunizations desk. No PCM visit is necessary to fill out this form.

🏃 Sports physical:
1. Required GA preparticipation physical evaluation form:http://ghsa.net/sites/default/files/documents/forms/2023_GHSA_PPE_Fillable_-_Revised_3-9-23.pdf
2. If your child had a Well Child visit within the last 12 months and the visit is documented in the MHS Genesis health record, an in person visit is NOT required. Complete pages 1-2 in the required form and drop it off with PCM team or email it via Patient Portal. PCM teams will complete forms within 3 duty days.
3. If your child has not had a Well Child visit in the last 12 months, an in person appointment is required. Contact PCM team for appointment and bring completed required forms.

As we wrap up Month of the Military Child, we want to recognize these two awesome military kids!

Kendall Vandersee, 5th grade, recently won 2nd place for the Houston County School District 5th Grade Oratorical Contest! There are over 3,000 5th graders in the district, and only 18 students were sent to compete. Congratulations, Kendall!

Tayla Coston, 12th grade, recently won the Military Youth of the Year for the Boys & Girls Clubs of America here at our Youth Center. Tayla went through a competitive selection process, where she was able to receive a scholarship as well. Congratulations, Tayla!

They were recognized by Col. Keyra Reyes, individual mobilization augmentee to the 78th Air Base Wing deputy commander.

Kendall and Tayla represent excellence and resiliency as military children. They go through a lot of challenges and sacrifices as their parents serve our nation, and we are so proud to have them as our family here at Team Robins.

💜Today, we wear purple to show military-connected children they’re seen, supported and celebrated.

Here at Robins AFB and all around the world, military kids show up — in classrooms, on teams, in clubs, and in their communities — bringing strength, resilience and heart wherever they go.

These children did not sign up to be in the military, but they were born into the military way of life. Today we Purple Up to honor their sacrifices because they deserve to feel the support of the Team Robins and the community around them.💜

Since 1987, the military has celebrated military children during the month of April. There are more than 1.6 million military children who face many challenges and unique experiences as a result of their parents’ service.

Hope to see a lot of our military families this evening's at the City of Warner Robins Purple Up Food Truck Roundup!💜

💜🎉In honor of the Month of the Military Child, the City of Warner Robins invites military families to join them for their Hometown Heroes: Military Kids Food Truck Roundup on Thursday, April 16, from 6:00 pm - 9:00 pm at Perkins Field.

The evening will include live entertainment, food trucks, resource tables, and family-friendly activities.

Hope to see everyone out there as we show our support for our amazing military kids. And don't forget to Purple Up! 💜
