We're Hiring: Home Care Services Coordinator!

Linked to Life is seeking a Home Care Services Coordinator to join our team. This 12-month, fixed-term role offers 28 hours per week, coordinating and overseeing home care services to ensure clients receive tailored support.

Key Responsibilities:
Coordinate Home Care Packages and CHSP services
Develop care plans and manage client budgets
Supervise and support care staff
Maintain strong relationships with clients, contractors, and professionals.

Essential Skills:
Experience with older people and Home Care Package coordination
Knowledge of My Aged Care and service delivery principles
Excellent communication and organizational skills
Valid driver’s license and own vehicle

As a Care Worker with us, you will provide essential services to our clients, enabling them to live independently and comfortably at home. Your duties will include:
• Personal Care – Assisting clients with daily tasks to maintain their dignity and independence.
• Domestic Cleaning – Supporting clients with household chores to ensure their homes are safe and clean.
• Social Support – Assisting clients with transportation to medical appointments, shopping, or social outings.
• Meal Preparation – Preparing nutritious meals to promote well-being.
