How to apply:

✉️ Complete and email it with proof of concession to [email protected]

🏢 Apply in person at the Shire Administration Building, 77 Windich Street
✅ Approved applicants receive a 30‑day voucher to book with
💉 Microchipping can be added for $44 if required
🐶 Finalise pet registration with the Shire after your appointment
