The Country SA PHN (CSAPHN) is the primary health network covering the country regions of South Australia. The Australian Government announced the establishment of Primary Health Networks, in 2014, and are to be known as PHNs. The key objectives of PHNs are to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and improve

coordination of care to ensure patients receive the right care in the right place at the right time. Our six key priority areas are; Aboriginal and Torres Strait Islander Health, Aged Care, Digital Health (eHealth), Health Workforce, Metal Health and Population Health. The CSAPHN works to bridge the gap of health inequity and access in rural South Australia by building a collaborative and responsive country SA health care system. Code of conduct:

Country SA PHN care finder lead Helen Morley recently travelled around Country South Australia to check in with our commissioned care finder service providers: iREACH Rural Health in Murray Bridge, Summit Health Health in Port Elliot and and Country & Outback Health Inc in Port Pirie.

She said, "to see staff in their own environment provides a greater context to understanding the regions for me. It builds a mutual connection and is especially meaningful when I see people are proud of the regional towns they live, and the organisations they represent. The incredible compassion the care finders have for supporting older people is inspiring to me every day!"
