Staying in Place Coordinator
⏰ 10 hours per week (part-time, very flexible)
🏢 Based at the Quairading CRC (ideally)
💰 SCHADS award pay rates apply
Supported by Tracey (Seniors Engagement Coordinator – new title!) and reporting to Jill (CRC Coordinator), this position will be responsible for coordinating the Staying in Place program through our Community Partnership with HomeMade. This position involves working with clients with "Support at Home" funding, connecting them with local Support Workers on Mable and other services and resources, all with an aim of keeping older community members living in their own homes as long as possible.
Preferred candidate attributes:
• Knowledge, awareness and understanding of the Staying in Place model, the Commonwealth Aged Care Home Support Program and Aged Care Quality Standards.
• Service planning and budget management experience.
• Experience using online platforms, spreadsheets and digital systems.
• Ability to develop genuine and lasting relations with community members of all ages.
• Excellent writing and communication skills.
• Patience and understanding toward all colleagues, customers, and community members.
