Community Gateway Community Gateway is an Aged Care and Disability Service Provider located in the Illawarra, Shoalhaven and Mid North Coast.

Our primary objective is to provide quality, accessible and empowering support to our service users. Other Objectives include:

- Individualised support and information is provided to enable service users to have informed choice.
- Service users have the right to self determination about support provided to achieve and maintain social independence.
- Service provision meets or exceeds all standard

s, guidelines and the professional ethics of the organisation.
- Valued partnerships with service users, representative groups and other stakeholders are built and maintained.
- Program outcomes are effective and meet all funding requirements.

Fly2Health practitioners are now co-locating with us at our Kempsey Hub fortnightly, helping bring more allied health support to our local community.

Currently, an Occupational Therapist is on site every Wednesday & Thursday, with appointments available for:

✔️ Aged Care assessments
