ADVISORY
The SSS on-site services in Saskatoon, Saskatchewan, will be extended from April 27 to April 28, 2026, 10:00 AM to 4:00 PM at La Familia Grocery.
Services will be rendered on a first-come, first-served basis. No required appointment.
To ensure a more efficient service experience and maintain high-quality service, all SSS transacting members are required to bring the correctly completed relevant forms upon their visit to save time, speed up the process, and create a smoother transaction.
For your reference, please check below applicable forms and basic requirements.
A. Member Data Change Request and Addition/Deletion of Beneficiaries
•Properly Accomplished two (2) MDCR Forms, download your form here:https://www.sss.gov.ph/wp-content/uploads/2024/10/E4-Member-Data-Change-Request.pdf
•For pensioner,https://www.sss.gov.ph/wp-content/uploads/2024/10/Pensioners-Data-Change-PEN01439.pdf
•Original and Photocopy of Passport or two (2) valid IDs.
Other applicable documents/information as required based on your request can be found on the back page of the MDCR form.
B. Benefit Claim Inquiries (Maternity, Sickness, Disability, Retirement, Funeral, Death)
Basic requirements: Original and Photocopy of Valid Passport Only
•Any other requirements will be provided upon interview and verification of records
•Application forms shall be provided by SSS Representative. DO NOT accomplish any form unless with previous instructions from SSS.

C. Annual Confirmation of Pensioners (ACOP) / Yearly Appearance
•Original passport or two valid IDs. No required photocopy

The relevant forms and instructions for any other transactions or requests not mentioned above will be provided by SSS during your visit. Please prepare the original and a photocopy of your passport or two valid IDs.
