⚠️ Attention Community Pharmacies – Preventable Allergy Event!

It is important to review the patient profile for all prescriptions received to check for drug interactions or allergies.

A recent incident reported to the NIDR described a patient who experienced an allergic reaction to an over the counter (OTC) product. The pharmacy received a faxed prescription from the prescriber but did not process it in the pharmacy software. The pharmacy missed the documented allergy on file, and the patient experienced harm. This event could have been avoided, had the patient’s profile been reviewed before the medication was provided.
