📚 A valuable resource for psychodynamic diagnosis: OPD-3

We would like to draw your attention to the Operationalized Psychodynamic Diagnosis (OPD-3) – a comprehensive framework for psychodynamic diagnosis and treatment planning.

OPD-3 offers a structured way of thinking about patients beyond symptom-based diagnosis. It considers psychodynamic conflicts, personality structure, interpersonal patterns and the patient’s experience of illness, helping clinicians develop a deeper and more clinically meaningful understanding of the person.

The manual can be particularly useful for psychotherapists who want to connect psychodynamic formulation with systematic diagnostic thinking and treatment planning.

The editors: OPD Taks Force, the Working Group for Operationalized Psychodynamic Diagnostics (OPD), consisting of psychiatrists, psychoanalytic and psychodynamic psychotherapists, and psychosomatic specialists from German speaking countries, developed the OPD in the early 1990s. Its goal was to create a multi-axial, reliable, and validatable diagnostic instrument for psychodynamic therapy approaches in order to improve the planning and evaluation of therapy and to standardize psychodynamic diagnostics and make them suitable for research.

☘️Perversion in the couple. A psychoanalytic evaluation?

In this paper, the author describes clinical cases that have allowed them to reflect on the functioning of perverse couples. First of all, they distinguish the polymorphous functioning present in normal couples from that present in perverse couples. In the latter, they observe that the fact that these couples seek treatment reveals an ego-dystonic aspect of the pathology that differs from “proper” perversion. The fact that these couples were married differentiates them from perverse patients who, in analysis, show great difficulty in maintaining a stable link. This suggests that perverse couples are more accessible to therapy. The author therefore hypothesises that perversion within a couple has an underlying shared functioning, even if only one partner shows the proper symptoms. They also suggest that a perverse link within a couple can defend against breakdown and help the partners avoid awareness of having a devalued and rejected body. The author will also examine a dimension of the perverse link between couples, connected to the sharing of a folie à deux.

🌿In the last eddition of the EFPP E-journal 📖 we may find the full contribution the important information on
Psychotherapy Regulation in France 🇫🇷🧠

👉A contribution by 📌Odile Gavériaux and📌 Frank Bonelli explores the development of the legal framework for psychotherapy in France and highlights some important challenges for the profession.

🔍Until 2004, the title of psychotherapist was not legally regulated in France. Law 2004-806 introduced legal protection of the title and established registration requirements for professionals, including doctors, psychologists and registered psychoanalysts. The regulation was initially motivated largely by the 👉need to prevent abuse and sectarian practices rather than to establish a comprehensive framework for the psychotherapy profession.

🖊️A 2010 decree subsequently introduced specific requirements in clinical psychopathology, including
👉400 hours of theoretical training and
👉a minimum five-month clinical internship.
However, the framework does not constitute a specific psychotherapy training and gives a prominent place to approaches such as CBT, cognitive and psycho-educational methods, and EMDR.

The French experience also illustrates the growing influence of health insurance and state policy on psychotherapy. The Mon Soutien Psy programme, introduced to improve access to psychological care, has expanded the availability of reimbursed psychological support – but currently remains focused on registered psychologists rather than psychotherapists.

Perhaps most significantly, the debate around Amendment 159 highlighted concerns about the future of psychoanalytic approaches within publicly funded mental healthcare. A proposal to end health-insurance reimbursement for psychoanalytically based care was ❌ultimately rejected. The debate raised fundamental questions about therapeutic pluralism, patient choice, professional autonomy and the role of the state in determining which therapeutic approaches are recognised and supported.

The French case therefore raises questions that extend far beyond France:

🔹 Who should define what constitutes legitimate psychotherapy?
🔹 How can regulation protect patients without restricting therapeutic pluralism?
🔹 What place should psychoanalytic psychotherapy have within publicly funded healthcare?
🔹 How do we balance evidence-based practice with clinical autonomy and individual patient needs?

These questions are central to the ongoing work of the EFPP Regulation Working Group as it compares regulatory frameworks across Europe and considers how psychoanalytic psychotherapy can maintain a strong professional and ethical voice.
