Have you, or someone you support, had a delayed or difficult experience leaving hospital? If yes - we want to hear from you 💬

Who is this survey for? We want to hear from people who had a difficult experience leaving hospital in the last 12 months, and from carers, relatives or friends who supported them.

This could include:
✅ A delay leaving hospital
✅ Leaving hospital before you felt ready
✅ Leaving before the right care, support, equipment, medication, transport, housing or follow-up arrangements were in place
✅ Something else made leaving hospital difficult
