We have recently had the pleasure of working alongside Nottinghamshire Healthcare NHS Foundation Trust, supporting a medication reduction plan for a person we support, in line with STOMP (Stopping over medication of people with a learning disability, autism or both). Subsequently, we were then asked to present a case study to a group of professionals, to help inform practice in the future, and highlight how this partnership has assisted the medication reduction plan.
More importantly, we wanted to highlight the significant impact this has had on the person’s life, along with helping their family to feel assured that their loved one is receiving the care they need and deserve.
Simply put, the person’s family said “from my perspective, it looks like they have less weight on their shoulders. A very positive direction”
Alison Barr, Principal Pharmacist at Nottinghamshire Healthcare also said:
“The Trust, CAMHS and Pharmacy teams have provided close support and advice every step of the way whilst we slowly reduced medicines for this patient, working closely with parents and care providers. Seeing the impact this has had on the quality of life for the patient is fantastic and is an example of just how important it is to regularly review patients and their medicines and personalise the care they receive.
