Victoria Community Care Ltd is an awarding winning care provider with Investors in People accreditation.
We are looking for a care co-ordinator to join our excellent team of co-ordinators. No previous experience necessary as full training will be given but experience in care, access to a vehicle and full driving licence is essential.
This is a full time permanent post at 37.5hrs per week and salaried.
The shifts will be 8am - 2pm / 4pm-10pm shifts over a 7 day week covering over a four weekly rota including alternative weekends.
Your primary role is to organise and co-ordinate care services in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the Manager.
Main Duties
• Ensure that the care service is delivered to service users at the times and for the duration specified in the relevant individual agreements and contract standards.
• Ensure that the staff allocated, and the staff attending, each service user have the relevant skills, knowledge and resources related to the wishes and needs of that service user.
• Ensure that each staff member is introduced to the service user before or at the beginning of the first visit by that staff member, and that the service user always knows who to expect at each visit.
• Manage travel delays and staff shortages to mitigate to the fullest extent possible the impact on the service user, managing and minimising their distress and insecurity in such situations.
• Assist in the development of the philosophy, goals and objectives for the care practice.
• Assist in the assessment of the effectiveness of care implementation and delivery.
• Implement action to meet and maintain care standards.
• Work in cooperation with members of multi-disciplinary health teams so as to maximise opportunities for service user therapeutic care.
• Ensure that the care service is delivered in a safe and person centred manner.
• Ensure that service user rights are protected.
• Support and maintain a culture of performance and excellence.
