UPDATE: WELSH GENDER SURGERY PAUSE: PATIENT SAFETY MUST COME BEFORE POLITICS
New official information has now revealed why the Welsh Gender Service’s surgical pathway was placed under Level 3 escalation.
The NHS Wales Joint Commissioning Committee says checks carried out during July and August identified:
● Surgical referral rates from Wales that were significantly higher than those from English gender clinics.
● A lower proportion of Welsh patients progressing to some operations after referral.
● Concerns about whether parts of the pre-referral assessment process had been completed appropriately.
● Questions about whether some patients met basic surgical requirements, including BMI and overall surgical fitness, and whether their wider health needs had been properly considered.
These are serious concerns—but they remain concerns under investigation, not completed findings.
An urgent independent clinical review will now examine patients on the surgical waiting lists. The review is to be undertaken by experienced gender clinicians.
Following my correspondence to the First Minister raising concerns about referral rates and clinical oversight, I welcome the precautionary action. However, pausing surgery is only the beginning. The Welsh Government and NHS Wales must now provide full transparency.
As of 7 September, they have still not published:
• The review’s full terms of reference.
• The names, qualifications and declared interests of the reviewers.
• Its detailed scope, evidence standards and methodology.
• How many patients are affected.
• Its expected completion and publication dates.
• The conditions that must be satisfied before the pause can be lifted.
This is an all-Wales pause affecting the surgical pathway, including patients referred from areas served by Cwm Taf Morgannwg and Swansea Bay health boards. It does not mean that their local gender teams have closed. Hormone, endocrine, psychological, speech and language, fertility and other non-surgical support continues.
NHS Wales also says necessary follow-up for people who have undergone surgery, treatment for complications, aftercare and planned multi-stage procedures will not be affected. Patients should attend existing Welsh Gender Service appointments unless contacted directly and told otherwise.
This issue must never become an excuse to attack or stigmatise transgender people. Every patient—whatever their background or identity—is entitled to safe, evidence-based treatment, robust assessment and honest information.
Patient welfare, clinical safety and public accountability must come first.
