Marine Accident Investigation Branch The MAIB examines and investigates all types of marine accidents to or onboard UK ships worldwide The MAIB is part of the Department for Transport.

The Branch was established following the Herald of Free Enterprise disaster. The Herald of Free Enterprise, a UK flagged roll-on roll-off ferry, flooded and subsequently capsized shortly after leaving the Belgium port of Zeebrugge on the 6 March 1987, with the loss of 193 passengers and crew. A Formal Investigation was carried by order of the Secretary of State. One of the outcomes of the investigation was the formation of the MAIB in 1989.

Today is National Fishing Remembrance Day - a day to remember those in the UK fishing industry who have lost their lives, and to reflect on the importance of improving safety at sea.

We're marking this day by sharing news that the wreck of Nicola Faith has been donated to Cranfield University, where it will be used to train future accident investigators.

Nicola Faith was a fishing vessel that capsized off the coast of Colwyn Bay, Wales in 2021 leading to the tragic loss of the three crew members. By using the vessel to recreate realistic investigation scenarios, we hope the vessel can help improve safety for fishers and play a part in preventing future tragedies.

Are you prepared for a man overboard emergency?

* Maintain equipment: ensure crew know how to use safety equipment that is fit for purpose and can recover an unconscious person from the water.

* Carry out man overboard drills: regularly practise emergency drills so crew are fully prepared.

* What’s plan B? Consider alternative methods of recovery in advance.

Remember: separate from shooting gear at all times, manage risk and wear a PFD.

Today, we've published our report and safety flyer on the investigation of the foundering of the fishing vessel Freedom II 11 nautical miles south-west of Oban, Scotland on 21 February 2024.

* Excessive vibrations can fracture pipes and lead to floods. Be prepared and isolate all seawater valves if in doubt of the flooding source.

* The skipper conducted inductions and drills for their crew – these save lives.

* Regularly check for progressive flooding into adjacent compartments and adjust your plan appropriately.

* Press the button - use preformatted digital selective calling to raise the alarm.
