সংশোধিত ড. আম্বেদকর মেডিক্যাল এইড স্কিমে তফসিলি জাতি-উপজাতির রোগীদের চিকিৎসা সহায়তার পরিধি বাড়ল
হাইলাকান্দি, ১৬ আগস্ট:---গুরুতর ও জীবনসঙ্কটাপন্ন রোগে আক্রান্ত তফসিলি জাতি (SC) ও তফসিলি উপজাতি (ST)-র রোগীরা এ বার সংশোধিত ড. আম্বেদকর মেডিক্যাল এইড স্কিম-এর আওতায় আর্থিক সহায়তা পেতে পারেন। চিকিৎসার পরিধি বাড়ানোর পাশাপাশি এই প্রকল্পের আওতাভুক্ত হাসপাতালের সংখ্যাও উল্লেখযোগ্য ভাবে বাড়ানো হয়েছে। ফলে ব্যয়বহুল অস্ত্রোপচার, ক্যানসার চিকিৎসা, ডায়ালিসিস এবং অঙ্গ প্রতিস্থাপনের মতো চিকিৎসার ক্ষেত্রে সংশ্লিষ্ট পরিবারগুলি বিশেষ ভাবে উপকৃত হতে পারে।
কেন্দ্রীয় সামাজিক ন্যায় ও ক্ষমতায়ন মন্ত্রকের অধীন ড. আম্বেদকর ফাউন্ডেশন (DAF)-এর মাধ্যমে পরিচালিত এই প্রকল্পটি ২০০৪ সালের জানুয়ারি থেকে চালু রয়েছে। চলতি বছরের ২ জুন জারি করা সংশোধিত নির্দেশিকা অনুযায়ী, বার্ষিক পারিবারিক আয় সর্বোচ্চ ৫ লক্ষ টাকা পর্যন্ত হলে কিডনি, হৃদ্যন্ত্র, লিভার, ক্যানসার ও মস্তিষ্কের গুরুতর রোগের চিকিৎসা-সহ অঙ্গ প্রতিস্থাপন, মেরুদণ্ডের অস্ত্রোপচার এবং অন্যান্য জীবনসঙ্কটাপন্ন রোগের চিকিৎসার জন্য আর্থিক সহায়তার আবেদন করা যাবে।
কিডনির অস্ত্রোপচার বা ডায়ালিসিস এবং কিডনি বা অন্য অঙ্গ প্রতিস্থাপনের ক্ষেত্রে সর্বোচ্চ ৩.৫ লক্ষ টাকা পর্যন্ত সহায়তা দেওয়া হবে। ক্যানসারের অস্ত্রোপচার, কেমোথেরাপি ও রেডিয়োথেরাপির জন্য সর্বোচ্চ ২.৫ লক্ষ টাকা পর্যন্ত সহায়তা মিলতে পারে। মস্তিষ্কের অস্ত্রোপচার এবং অন্যান্য জীবনসঙ্কটাপন্ন রোগের ক্ষেত্রে সহায়তার ঊর্ধ্বসীমা ২ লক্ষ টাকা। হৃদ্যন্ত্র ও মেরুদণ্ডের অস্ত্রোপচারের ক্ষেত্রে সর্বোচ্চ ১.৫ লক্ষ টাকা পর্যন্ত আর্থিক সহায়তা পাওয়া যাবে।
সংশোধিত প্রকল্পে চিকিৎসার জন্য নির্ধারিত হাসপাতালের তালিকাও উল্লেখযোগ্য ভাবে সম্প্রসারিত করা হয়েছে। নয়াদিল্লি ও অন্যান্য রাজ্যের AIIMS-এর পাশাপাশি লখনউয়ের সঞ্জয় গান্ধী পোস্ট গ্র্যাজুয়েট ইনস্টিটিউট, পাটনা মেডিক্যাল কলেজ হাসপাতাল, জবলপুর হাসপাতাল অ্যান্ড রিসার্চ সেন্টার, গুয়াহাটির বি. বরুয়া ক্যানসার ইনস্টিটিউট, কলকাতার বিড়লা হার্ট ফাউন্ডেশন, ভুবনেশ্বরের কলিঙ্গ হাসপাতাল, মুম্বইয়ের টাটা ক্যানসার রিসার্চ ইনস্টিটিউট, হায়দরাবাদের নিজামস ইনস্টিটিউট অফ মেডিক্যাল সায়েন্সেস এবং চেন্নাইয়ের দ্য ভলান্টারি হেলথ সার্ভিসেস-সহ একাধিক প্রতিষ্ঠান এই প্রকল্পের আওতায় রয়েছে।
এ ছাড়াও CGHS অনুমোদিত হাসপাতাল, রাজ্য সরকারের মেডিক্যাল কলেজ ও তাদের সংযুক্ত হাসপাতাল, রাজ্য হাসপাতাল, রাজ্য সরকারের স্বীকৃত হাসপাতাল এবং সম্পূর্ণ ভাবে কেন্দ্র বা রাজ্য সরকারের অর্থপুষ্ট হাসপাতালগুলিও এই প্রকল্পের আওতায় রয়েছে। জেলা সদর ও বড় শহরগুলিতে বড় অস্ত্রোপচার এবং জীবনসঙ্কটাপন্ন রোগের চিকিৎসার প্রয়োজনীয় পরিকাঠামো থাকা সরকারি হাসপাতালগুলিও এই সহায়তা প্রকল্পের অন্তর্ভুক্ত।
বড় অস্ত্রোপচারের খরচ বহন করতে সমস্যায় থাকা পরিবারগুলির জন্য প্রকল্পটির অন্যতম গুরুত্বপূর্ণ সংস্থান হল, অনুমোদিত চিকিৎসার আনুমানিক ব্যয়ের ১০০ শতাংশ পর্যন্ত ফাউন্ডেশন সংশ্লিষ্ট হাসপাতালে সরাসরি প্রদান করতে পারে। তবে তা নির্ধারিত সর্বোচ্চ সীমার মধ্যে হতে হবে। RTGS, NEFT অথবা ডিমান্ড ড্রাফটের মাধ্যমে এই অর্থ প্রদান করা যেতে পারে।
তবে চিকিৎসার আগে আবেদন জমা দেওয়া বাধ্যতামূলক। প্রয়োজনীয় নথিপত্র-সহ সম্পূর্ণ আবেদনপত্র নির্ধারিত অস্ত্রোপচার, ডায়ালিসিস, কেমোথেরাপি বা রেডিয়োথেরাপির অন্তত ১৫ দিন আগে Director, Dr Ambedkar Foundation, 15 Janpath, New Delhi-110001-এর কাছে পৌঁছতে হবে।
আবেদনকারীদের জাতিগত শংসাপত্র, সাম্প্রতিক আয় শংসাপত্র এবং রেশন কার্ড অথবা আধার কার্ডের সঙ্গে স্ব-প্রত্যয়িত ছবি জমা দিতে হবে। সংশ্লিষ্ট হাসপাতালের মেডিক্যাল সুপারিনটেনডেন্টের দেওয়া চিকিৎসার আনুমানিক ব্যয়ের মূল শংসাপত্রও জমা দিতে হবে। ওই শংসাপত্রে প্রস্তাবিত চিকিৎসার নির্ধারিত তারিখ উল্লেখ থাকা আবশ্যক।
কিডনি বা অন্য কোনও অঙ্গ প্রতিস্থাপনের প্রয়োজন রয়েছে এমন রোগীদের জন্য সংশোধিত নির্দেশিকায় বিশেষ ব্যবস্থা রাখা হয়েছে। PM-JAY-এর আওতাভুক্ত উপভোক্তারাও কিডনি প্রতিস্থাপনের ক্ষেত্রে এই প্রকল্পের অধীনে সহায়তার জন্য আবেদন করতে পারবেন, কারণ কিডনি প্রতিস্থাপন PM-JAY-এর আওতায় নেই। তবে এর জন্য প্রকল্পে নির্ধারিত শর্তগুলি প্রযোজ্য হবে। দীর্ঘমেয়াদি রোগের ক্ষেত্রে পরবর্তী আর্থিক বছরে দ্বিতীয় বার নবীকরণ অনুদান দেওয়ার বিষয়টিও বিবেচনা করা হতে পারে।
আর্থিক সহায়তা পাওয়ার পর এক মাসের মধ্যে হাসপাতালকে সেই অর্থ ব্যবহার করতে হবে। রোগীকে হাসপাতাল থেকে ছাড়ার সময় ডিসচার্জ সার্টিফিকেট, চূড়ান্ত বিল এবং অর্থ ব্যবহারের শংসাপত্র ফাউন্ডেশনের কাছে জমা দিতে হবে। অব্যবহৃত অর্থ থাকলে তা ফেরত দিতে হবে। ফাউন্ডেশন স্পষ্ট করে জানিয়েছে, এই প্রকল্পের মাধ্যমে পাওয়া সহায়তা এবং অন্য কোনও উৎস থেকে পাওয়া আর্থিক সহায়তা মিলিয়ে চিকিৎসার মোট আনুমানিক ব্যয়ের বেশি হতে পারবে না।
উল্লেখ্য, সাধারণ ভাবে ইতিমধ্যেই সম্পন্ন হয়ে যাওয়া চিকিৎসার খরচ ফেরত দেওয়ার আবেদন এই প্রকল্পে গ্রহণ করা হয় না। তবে কোনও ক্ষেত্রে অস্ত্রোপচারের নির্ধারিত তারিখের অন্তত ১৫ দিন আগে আবেদনটি ফাউন্ডেশনে পৌঁছে থাকলে, বিষয়টি যোগ্যতার ভিত্তিতে বিশেষ ভাবে বিবেচনা করা হতে পারে।
অসমের রোগী ও তাঁদের পরিবারের কাছে এই সংশোধিত প্রকল্প বিশেষ ভাবে গুরুত্বপূর্ণ হয়ে উঠতে পারে। গুয়াহাটির বি. বরুয়া ক্যানসার ইনস্টিটিউটকে অন্তর্ভুক্ত করা এবং সরকারি ও স্বীকৃত হাসপাতালের বিস্তৃত নেটওয়ার্কের আওতায় চিকিৎসার সুযোগ থাকায় ক্যানসার-সহ বিভিন্ন জটিল ও ব্যয়বহুল চিকিৎসার ক্ষেত্রে রাজ্যের বহু দরিদ্র পরিবার উপকৃত হওয়ার সম্ভাবনা রয়েছে।
নির্ধারিত ফরম্যাটে অথবা ফাউন্ডেশনের নির্ধারিত বিন্যাস অনুসরণ করে সাদা কাগজেও আবেদন করা যাবে। আবেদনপত্র, সংশোধিত নির্দেশিকা এবং হাসপাতালের আনুমানিক ব্যয়পত্র, ব্যাঙ্ক ম্যান্ডেট ও ইউটিলাইজেশন সার্টিফিকেট-সহ অন্যান্য প্রয়োজনীয় নথির ফরম্যাট ড. আম্বেদকর ফাউন্ডেশনের সরকারি ওয়েবসাইটে পাওয়া যাবে।**

Coverage Expanded for SC/ST Patients Under Revised Dr. Ambedkar Medical Aid Scheme
Hailakandi, August 16: Patients belonging to Scheduled Castes (SC) and Scheduled Tribes (ST) suffering from serious and life-threatening illnesses can now receive financial assistance under the revised Dr. Ambedkar Medical Aid Scheme. Along with expanding the scope of medical coverage, the number of empanelled hospitals under this scheme has been significantly increased. Consequently, eligible families can benefit greatly in cases of expensive surgeries, cancer treatments, dialysis, and organ transplants.
Managed by the Dr. Ambedkar Foundation (DAF) under the Union Ministry of Social Justice and Empowerment, this scheme has been functional since January 2004. According to the revised guidelines issued on June 2 of this year, families with a maximum annual income of up to ₹5 lakh can apply for financial aid for serious ailments of the kidney, heart, liver, brain, cancer, organ transplants, spinal surgeries, and other life-threatening conditions.
Financial Assistance Limits
Kidney Surgery, Dialysis, or Organ Transplants (Kidney/Other Organs): Maximum up to ₹3.5 lakh
Cancer Treatment (Surgery, Chemotherapy & Radiotherapy): Maximum up to ₹2.5 lakh
Brain Surgeries & Other Life-Threatening Ailments: Maximum up to ₹2 lakh
Heart & Spinal Surgeries: Maximum up to ₹1.5 lakh
Empanelled Hospitals Network
The list of designated hospitals for treatment under the revised scheme has been substantially widened. It includes AIIMS New Delhi and other state AIIMS, Sanjay Gandhi Postgraduate Institute (Lucknow), Patna Medical College Hospital, Jabalpur Hospital and Research Centre, B. Borooah Cancer Institute (Guwahati), Birla Heart Foundation (Kolkata), Kalinga Hospital (Bhubaneswar), Tata Cancer Research Institute (Mumbai), Nizam’s Institute of Medical Sciences (Hyderabad), and The Voluntary Health Services (Chennai).
Additionally, CGHS-empanelled hospitals, state government medical colleges and their affiliated hospitals, state hospitals, state government-recognized hospitals, and hospitals fully funded by the Central or State Governments are included. Government hospitals in district headquarters and major cities equipped with necessary infrastructure for major surgeries and critical care are also covered.
Direct Payment System
To assist families struggling with the high costs of major surgeries, a key feature of the scheme is that the Foundation can directly pay up to 100% of the estimated treatment cost directly to the concerned hospital, capped at the maximum applicable limit. Payments are processed via RTGS, NEFT, or Demand Draft.
Application Process & Guidelines
Prior Application Required: Applications must be submitted before treatment begins. Complete application forms with required documents must reach the Director, Dr. Ambedkar Foundation, 15 Janpath, New Delhi-110001 at least 15 days prior to the scheduled date of surgery, dialysis, chemotherapy, or radiotherapy.
Required Documentation: Applicants must submit a caste certificate, recent income certificate, a self-attested photo along with a Ration Card or identity card, and the original cost estimate certificate issued by the Medical Superintendent of the concerned hospital (clearly stating the scheduled date of treatment).
Special Provisions: Beneficiaries under PM-JAY can also apply for kidney transplants under this scheme, as kidney transplants are not covered under PM-JAY (subject to scheme conditions). Provision for a second/renewal grant in subsequent financial years may be considered for long-term treatments.
Utilization Rules: Hospitals must utilize the funds within one month of receipt. Upon discharge, the hospital must submit the discharge certificate, final bill, and utilization certificate to the Foundation. Any unutilized funds must be refunded. Assistance combined from this scheme and other sources cannot exceed the total estimated cost of treatment.
Retroactive Claims: Generally, reimbursement for already completed treatments is not entertained. However, if the application reached the Foundation at least 15 days prior to the scheduled surgery date, it may be evaluated on a case-by-case basis based on merit.
This revised scheme is particularly beneficial for patients in Assam. The inclusion of B. Borooah Cancer Institute in Guwahati and the broad network of government and recognized hospitals will provide major relief to low-income families in the region facing complex and costly medical treatments.
Application forms, revised guidelines, standard formats for hospital estimates, bank mandates, and utilization certificates are available on the official website of the Dr. Ambedkar Foundation.
