ADARSH PRATISHTHAN is a group of citizens from the town of Shahada, who decided to get together and begin a movement in their small way towards sustainable and wholesome development of the society. No matter how small a difference this endeavor would make towards solving the magnanimous problems that we face, this group is determined to strive towards fulfillment of their dream … A dream of a land

that is proud of its culture, a land where children grow to their full potential and prosper to being successful, purposeful and responsible citizens of the future. The aim for starting this non government organization was to in a way give back to the society a part of what it has so patiently bestowed upon us over the years gone by. Our focus today remains on Malnutrition amongst the tribal children in the “Satpuda” mountain ranges. These children have far been brought up in an environment of ignorance, illiteracy and sheer disregard for human life and more is the life of a child.

Mangal &Pariwar.
Magal, 3 year old boy weighing ( 5.9kg!) was got to our pediatrician's OPD by a distressed, worried and exhausted Health worker to get him admitted for Severe Acute Malnutrition with ulcers in the left eye! She said she went to their house everyday for 1 month to convince this grandmother to take him to the district hospital. Granny refused, as she had already been to the NRC at dist hospital but wasn't satisfied!
At last the HW convinced her to get the child admitted in Kulkarni Hospital under Adarsh Pratishthan AdarshPratishthan Shahada .
When Mangal arrived, he was in a very bad state, as can be seen. It took a lot of convincing, cajoling, a little intimidating to convince the granny to stay,( for free treatment, free stay and free medicines!)
Mangal was treated in the PICU for 12 days, with iv medications, blood transfusion, feeds, eye drops, vitamin supplements etc.
At times one felt anger, frustration, helplessness looking at Mangal.
But Mangal had attitude! He would only eat what he felt was tasty. For him, tasty was only bajra bhakri(as that as the only meal he had recieved)
Granny was initially skeptical but later started taking good care, but all our angry young Mangal ever cared for was his older brother. Absolutely loved being with his brother.
Slowly Mangal's condition improved, though we couldn't save his left eye, vision in right eye was good.
Discharged on 18th day on full diet, Mangal had come for 1st follow up after 15days of discharge, happy and spunky.
As a citizen of an aspirational country that visualizes itself as a super power, I feel ashamed, helpless, angry that many such Mangals live and die in our country without any tear being shed!
I am grateful we could save this Mangal, with his family's help, today he has learnt to smile again, but was this enough??
We are proud of our Mangalyans, and rightly so, but let's pay heed to our Mangals, who so evidently want to live and prosper!

Here is one more good news for one and all.Another story of struggle, worry and ultimately smiling faces and thankful goodbyes
This story is about prithviraj vasave, a five month old brought to our opd with loose motions. Our pediatrician Dr Alaka Ashtaputre Kulkarni noticed that the child was coughing continuously. On asking leading question about the cough ,the mother complained that this malnourished child coughed throughout the day." But now he has loose motions " she said haplessly.
Routine lab work done and chest x-ray done .
X ray revealed a rare congenital problem. "Congenital lobar emphysema"(CLE). We wondered how the child survived a CLE till five months of age!Now the issue again was it needed urgent surgical intervention or else this lung would burst and lead to catastrophic complications in the baby.
But it was anemic, malnourished and also had diarrhoea. His father was away in gujrath as a migrant labour.
We decided to stabilize the child. Treated the diarrhoea, which took a while! Transfused blood and waited for the father to arrive. Treated and fed him food after recovery from the viral diarrhoea.
The parents were relieved that we could give them athe cause for the cough, but again neither did they have the money or the courage to go to a tertiary centre for the operation.
Hence again we had to request our able surgical team, Dr Darshan Kulkarni (surgeon) and Dr Chandrakant Patil to operate on this patient as a genuine charity. We explained that the medical and surgical expences would be borne by adarsh pratisthan but we would not be able to pay them any fees. The doctors obliged and the child was operated upon on the 5th of February.
Both doctors put their busy schedules and lists aside to save this child that to for no charges.
This is a major pediatric surgery but the first one to be done in our tribal district.
Post operatively the patient recoverd marvellously.
Our pediatrician would keep questioning the mother "khwari avtolo ke "(does he have cough) and she would always say NO!
The child was discharged on day 10of surgery after suture removal and guess what ? He had put on a good 800gms weight in mere 10days and would smile back at us too.
This smile is worth all the trouble!
We are thankful to our surgeon Dr Darshan Kulkarni and our anesthetist Dr Chandrakant Patil, both of whom cancelled all their appointments for this smile on prithviraj's face.

9 months old Arvind Rathore was brought to Kulkarni Hospital with this huge mass in the neck (since birth) and weighed a mere 4 kg . Diagnosed as cystic Hygroma, this benign, fluid-filled congenital tumor was a huge mass in the neck, now causing breathlessness since 1 month. Parents were BPL card holders. He needed surgical intervention to remove the tumor. No existing government insurance covered this, nor were the parents ready to go to a tertiary level government Hospital. Hence Arvind was admitted under Aadarsh Pratishthan to kulkarni hospital, and operated upon on 21/12 /2017. (21st december).Surgery was done by our surgeon Dr. Darshan Kulkarni under general anesthesia. Anesthetist was Dr. Chandrakant Patil. Post operatively managed in PICU (ventilated and weaned) for 10 days, by anesthetist Dr. Chandrakant and pediatrician Dr. Malvika Kulkarni . The expenses for this surgery and post – op care in any private Hospital would range in a few lakh rupees. This was done under Aadarsh Pratishthan for a more expense to parents of Rs- 12500/-. Patient is discharged today after completion of antibiotics and post operative wound care. Here’s wishing Arvind a speedy recovery and Happy New Year to all of you!

we distributed these posters to all obstetric and pediatric hospitals in shahada. we got a mixed welcome. some doctors thanked us,some greeted us with a qussical brow. but that will not discourage us as our only intention is to spread the msg of importance of breast feeding. thankyou all doctors who supported us willingly or otherwise. we shall continue to seek your support for the cause.

With only 17 percent indian mothers breastfeeding their babies exclusively to 6 months we feel the need to promote breastfeeding; in such a away that women should feel proud of the fact that she breastfeeds her baby without inhibitions or concerns. encouraging breastfeeding will go a long way in curbing malnutrition. we at our headquarters at kulkarni hospital celebrate breast feeding with poster and slogan competing for the staff and patients.

SAM children fed gehuna (a type of therapeutic diet) by their mothers. we insist that they feed them in the dining area sitting together and encouraging discussion about diet. we have observed that this prctice increases their appetite.

all these children were admitted previously with severe acute malnutrition. now hopefully on their road to HEALTH!
