✨DETIK BERSEJARAH: TADBIR URUS BAHARU QA KKM DILULUSKAN! ✨

Satu detik bersejarah telah tercipta apabila syor penubuhan Jawatankuasa QA KKM baharu telah dimuktamadkan secara rasmi.

Di bawah tadbir urus baharu ini, sebanyak 37 inisiatif kualiti merentas bahagian/program akan dipantau secara bersepadu di bawah kepimpinan KPK. Kesemua inisiatif ini dihimpunkan di dalam tiga (3) Kumpulan Kerja Strategik (KKS) mengikut bidang fokus masing-masing yang akan dipengerusikan secara bersama oleh kesemua Timbalan Ketua Pengarah Kesihatan (TKPK).

Mesyuarat yang dipengerusikan oleh YBhg. Datuk Dr. Mahathar Bin Abdul Wahab, Ketua Pengarah Kesihatan (KPK) KKM dan dihadiri oleh barisan pengurusan tertinggi dan pimpinan atasan KKM itu telah meneliti dan seterusnya bersetuju untuk meluluskan struktur tadbir urus tersebut.

Penstrukturan baharu ini menjadi titik tolak transformasi tadbir urus inisiatif kualiti KKM dalam meningkatkan kecekapan pemantauan. Melalui integrasi ini, penyelarasan dapat dilaksanakan secara lebih bersepadu dan berasaskan kolaborasi merentas bahagian, sekali gus memantapkan kerangka Quality Assurance Programme (QAP) yang telah bertapak lebih daripada empat dekad lalu.

Institut Penyelidikan Sistem Kesihatan (IHSR) selaku Sekretariat QA yang telah diberi mandat untuk menyelaras dan menerajui usaha penstrukturan tersebut sejak Disember 2025 telah mengadakan beberapa siri sesi libat urus dan mesyuarat bersama 26 bahagian serta institusi, dengan penglibatan lebih 60 orang pegawai pada setiap sesi.

Usaha selama hampir lapan bulan tersebut telah berjaya dinoktahkan pada sesi mesyuarat tersebut dengan kerjasama serta komitmen yang diberikan oleh pegawai-pegawai penyelaras inisiatif kualiti di peringkat bahagian dan institusi serta QA Advocates.

Kelulusan ini akan disusuli dengan sesi libat urus bersama semua sekretariat KKS sebagai persediaan sebelum mesyuarat pertama diadakan pada penghujung tahun ini.

Three Years of Collaboration, One Meaningful Milestone!👣

Today marks the final stage of our project, “Quality of Foot Care Among People with Diabetes: A Malaysian Diabetic Foot Self-Care Assessment Instrument,” with the completion of the Final Stakeholder Presentation.

Over the past three years, IHSR and Occupational Therapists have worked closely together to develop a Malaysian instrument to assess diabetic foot self-care practices. This journey has been shaped by collaboration, constructive discussions, shared learning, and continuous refinement - all towards developing an instrument that is relevant, practical, and meaningful for diabetes care in Malaysia.

Today, we reached an important milestone as we brought together key stakeholders from the Disease Control Division, Family Medicine Specialists fraternity, Occupational Therapy profession, and the Allied Health Sciences Division. The session was chaired by Dr Muhammad Fadhli bin Mohd Yusoff, Director of IHSR, who provided guidance and support as stakeholders reviewed M-DiFoSC and shared valuable insights towards its implementation.

After three years of dedication and teamwork, the instrument now has its identity: M-DiFoSC - Malaysian Diabetic Foot Self-Care Assessment Instrument. Our heartfelt appreciation goes to everyone who has contributed their expertise, time, and commitment throughout this journey. Every discussion, suggestion, and refinement has played an important role in shaping M-DiFoSC.

We hope M-DiFoSC will serve as a practical and useful instrument for healthcare professionals to better understand patients’ foot self-care practices, identify gaps, and support targeted interventions, ultimately contributing towards better foot care and health outcomes for people living with diabetes in Malaysia.

From Zero to Champion: Implementing Quality Workshop (Emergency Medicine)
📅 28–30 July 2026

Upon the invitation from the Emergency Medicine Specialty through Dr Nor Khatijah Ahmad, CHQR collaborated to deliver the Zero to Champion workshop in support of the Emergency Medicine Parallel Pathway Programme (FRCEM Malaysia). The workshop aimed to strengthen Quality Improvement (QI) competencies among trainees while fostering a stronger culture of quality improvement across emergency services nationwide. Building on the successful collaboration with the Restorative Dentistry Specialty last April, this workshop marks another step in expanding QA/QI capacity-building across specialty training programmes.

Over three intensive days, 50 medical officers and emergency physicians from hospitals across the country came together to strengthen their knowledge and practical skills in Quality Assurance (QA) and Quality Improvement (QI). Through interactive lectures, group discussions, hands-on exercises and project presentations, participants learned the fundamentals of quality improvement—from identifying opportunities for improvement and analysing problems to developing key measures, designing effective interventions and planning sustainable quality initiatives.

A heartfelt appreciation to our dedicated trainers for sharing their expertise and guiding participants throughout the workshop: Mr (Dr) Azmi Alias, Dr Cheah Wee Kooi, Dr Farizan Abdul Ghaffar, Dr Samsiah Awang, Dr Nur Ezdiani Mohamed, and Dr Izzatur Rahmi Mohd Ujang.

A special thank you to all participants for your enthusiasm, active engagement and commitment throughout the workshop. Your willingness to learn, collaborate and share experiences made every session meaningful.

Bengkel Latihan Pengurusan Kes Gigitan Haiwan Berisiko Rabies dan Seminar One Health 2026 telah berjaya dianjurkan secara bersama oleh Sektor Zoonosis, Kementerian Kesihatan Malaysia (KKM), Institut Penyelidikan Sistem Kesihatan (IHSR), NIH dan Institut Penyelidikan Perubatan (IMR), NIH pada 20–22 Julai 2026.

Hari 2 & 3: Aplikasi Sains Pelaksanaan (Implementation Science)
Peserta didedahkan kepada aplikasi Implementation Science dalam memperkukuh pendekatan One Health melalui:
• Survelan bersepadu
• Pengurusan wabak
• Pengenalpastian jurang pelaksanaan
• Penyelesaian yang sesuai dengan konteks tempatan🔬

📊 Impak & Pencapaian Utama:
✅ Seramai lebih 120 orang pegawai dari pelbagai Jabatan Kesihatan Negeri (JKN) telah berjaya dilatih.
✅ Kadar kelulusan ujian pasca-latihan amat memberangsangkan iaitu melebihi 90%.
✅ Mewujudkan jaringan kerjasama taktikal bersama para pelaksana di lapangan bagi memastikan kerangka teori dapat diterjemahkan kepada tindakan serta impak nyata dalam sistem kesihatan negara.

MESYUARAT TECHNICAL WORKING GROUP BAGI PEMANTAUAN PELAKSANAAN NATIONAL POLICY FOR QUALITY IN HEALTHCARE (NPQH) BIL. 1/2026

Seramai 40–50 ahli Technical Working Group (TWG) serta wakil pelbagai inisiatif penambahbaikan kualiti (Quality Improvement Initiatives (QII)) Kementerian Kesihatan Malaysia telah berkumpul bagi meneliti kemajuan pelaksanaan Pelan Strategik di bawah National Policy for Quality in Healthcare (NPQH).

Mesyuarat tahunan ini menjadi platform penting untuk membentangkan pencapaian pelaksanaan melalui indikator pemantauan yang digariskan dalam polisi. Sebanyak 37 indikator dipantau bagi tahun 2025 dan dibentangkan untuk menilai kemajuan polisi terutamanya aktiviti-aktiviti strategik yang diutamakan ke arah memperkukuh sistem kualiti penjagaan kesihatan negara.

Antara pembentangan aktiviti yang diutamakan sepanjang mesyuarat ialah:

✅ Quality Improvement Competency for Healthcare Quality Professionals – A Scoping Review, yang bakal menjadi asas kepada Fasa 2 pembangunan Competency Framework bagi pegawai kualiti KKM.

✅ Cadangan modul dan laluan latihan pegawai kualiti, bagi memperkukuh pembangunan kompetensi dan kapasiti tenaga kerja kualiti.

✅ Analisis QHub 1.0, platform pembelajaran dalam talian yang menghimpunkan maklumat berkaitan QII di bawah KKM, termasuk sesi gerak kerja yang menghasilkan pelbagai input bernas bagi memperkasa pembangunan QHub pada fasa seterusnya.

✅ Cadangan pembangunan instrumen pengukuran Psychological Safety dalam kalangan kakitangan kesihatan sebagai langkah menyokong pembentukan budaya kerja yang selamat, telus dan berkualiti.

✅ Hasil analisis pengukuran kualiti sistem kesihatan menggunakan kerangka STEEEPA, termasuk perbincangan isu-isu strategik dan peluang penambahbaikan.

✅ Cadangan pelan semakan NPQH serta sesi pengisian maklum balas pelaksanaan polisi sebagai persediaan bagi semakan dan kemas kini dasar untuk tempoh seterusnya.

Lebih daripada sekadar sesi pelaporan pencapaian, mesyuarat ini menjadi medan strategik untuk menyelaras hala tuju, berkongsi pandangan dan pembelajaran merentas program serta memperkukuh kolaborasi dalam usaha memastikan setiap inisiatif kualiti diterjemahkan kepada penambahbaikan perkhidmatan yang memberi manfaat kepada rakyat.

Setiap indikator yang dipantau, setiap dapatan yang dibincangkan dan setiap cadangan yang dikemukakan merupakan sebahagian daripada komitmen berterusan Kementerian Kesihatan Malaysia untuk membina sistem kesihatan yang lebih S.T.E.E.E.P.A (SAFE, TIMELY, EFFECTIVE, EQUITABLE, EFFICIENT, PEOPLE-CENTRED and ACCESSIBLE).

What an incredible three days of leveling up our data skills!

🗺️ We just wrapped up our 3-day workshop, "QGIS: Basic Spatial Data Management and Analysis," which ran from 14-16 July 2026. Led by Centre for Health Equity Research(CHER), this course covered GIS fundamentals to basic map production. Participants learned how to load and manage spatial data, design thematic maps, and run real-world geoprocessing tasks, such as identifying catchment zones for healthcare service expansion.

It wasn't just theory, attendees dived right into hands-on sessions where they explored the QGIS interface, analysed spatial data clusters, and mastered layout managers to export professional-grade maps. This workshop was a fantastic opportunity for everyone to learn how to turn complex spatial data into clear, actionable visual insights. 📌MESYUARAT PEMURNIAN CADANGAN PEMERKASAAN STRUKTUR TADBIR URUS JAWATANKUASA QUALITY ASSURANCE (QA) KEMENTERIAN KESIHATAN MALAYSIA

Mesyuarat Pemurnian Cadangan Pemerkasaan Struktur Tadbir Urus Jawatankuasa QA KKM telah berjaya dilaksanakan. Berlangsung dalam dua sesi hibrid yang interaktif di NIH (30 Jun–1 Julai & 8 Julai 2026), mesyuarat ini merupakan satu pencapaian utama dalam usaha mentransformasi tadbir urus inisiatif kualiti kesihatan di peringkat Kementerian Kesihatan Malaysia.

- Mesyuarat Sesi 1 (30 Jun dan 1 Julai 2026) telah dipengerusikan oleh Dr. Muhammad Fadhli Bin Mohd Yusoff (Pengarah IHSR).
- Mesyuarat Sesi 2 (8 Julai 2026) telah dipengerusikan oleh YBhg. Datuk Dr. Nor Fariza Binti Ngah (Timbalan Ketua Pengarah Kesihatan, Penyelidikan & Sokongan Teknikal).

Susulan mandat yang diberikan oleh Ketua Pengarah Kesihatan KKM, IHSR selaku Sekretariat QA Kebangsaan telah menerajui serta memudah cara usaha merangka cadangan struktur baharu ini yang telah bermula sejak Disember 2025.

Mesyuarat menyaksikan penyertaan yang aktif dan komitmen yang tinggi daripada lebih 60 orang ahli, sama ada yang hadir secara fizikal mahupun maya, yang mewakili 26 bahagian dan program di seluruh KKM. Pihak Sekretariat merakamkan ucapan terima kasih atas input yang sangat berharga serta maklum balas yang membina dalam memurnikan lagi struktur yang dicadangkan!

⏭️Langkah Seterusnya?
Semua pandangan dan cadangan daripada sesi mesyuarat pemurnian ini akan diteliti dan diselaraskan bagi menambah baik draf cadangan struktur tadbir urus Jawatankuasa QA KKM tersebut. Draf yang telah dimurnikan akan dikongsi semula bersama ahli-ahli mesyuarat untuk semakan, sebelum dibentangkan secara rasmi kepada Ketua Pengarah Kesihatan untuk pertimbangan selanjutnya.

🌟 QHub Meets Risk Communication - Lecture Invitation on Quality Improvement Learning Hub (QHub): Risk Communication in Family Health Issues | Dewan Serbaguna 1, Hospital Tuanku Mukhriz, Cheras | 12 Jun 2026 🌟

CHQR, IHSR had the pleasure of engaging with almost 20 Semester 2 Doctor of Public Health (DrPH) students from UKM (Session 2025/2026) in a session on "Quality Improvement Learning Hub (QHub): Risk Communication in Family Health Issues" given by Dr Nur Ezdiani Mohamed. This was the 2nd invitation since 2025.

Coming from diverse public health backgrounds, the students were introduced to QHub—not merely as a repository of quality initiatives, best practices and experts, but as a platform that can facilitate knowledge sharing, collaboration and learning to address complex family health challenges. QHub was developed to strengthen knowledge translation, communication linkages, visibility and peer learning across healthcare quality initiatives, vertically and horizontally.
As part of the discussion, students were tasked to critically assess QHub using the WHO Strategic Communication Framework, evaluating how the platform fulfils the six communication domains and identifying opportunities to further strengthen its role in supporting evidence-informed decision-making and user engagement.
The session also challenged participants to think beyond traditional communication approaches and explore how platforms such as QHub can contribute to tackling family health issues. The most effective solutions often fail when they are not communicated, shared, understood or adopted. Conversely, meaningful communication can bridge silos, build trust, accelerate learning and transform local innovations into system-wide improvements.

One key message emerged from the session:

📢 Communication is not merely a supporting activity—it is a quality improvement intervention.
As future public health leaders, researchers and policymakers, we hope the students will continue to challenge conventional thinking and recognise communication as a strategic driver of healthcare quality, patient-centred care and population health outcomes and continue to leverage on the resources shared on QHub for the betterment of quality in healthcare service.

🌟 Closing the Gap, Building Champions: Zero to Champion National QA Training 2026 | 8 - 10 June 2026 | Ballroom, NIH 🌟

For almost three intensive and inspiring days, nearly 50 passionate healthcare professionals from across Malaysia gathered for the annual Bengkel Implementing Q: From Zero to Champion, organised by the Centre for Healthcare Quality Research (CHQR), Institute for Health Systems Research (IHSR).

Representing 10 teams comprising 9 State Health Departments and 1 Specialty Service, with specially invited participants from Hospital Al-Sultan Abdullah, UiTM Puncak Alam, the workshop served as a platform for learning, sharing and transforming ideas into actionable facility, district or speciality-driven quality improvement projects.
Participants were guided by a distinguished panel of QA/QI advocates, champions and judges — Mr (Dr) Azmi Alias (Consultant Neurosurgeon, HKL), Dr Cheah Wee KOOI (Geriatrician, Hosp Taiping), Pn Siti Rabiatul Adawiyah (Pharmacist, KK Rembau) together with the CHQR team. Through practical insights, real-world experiences, candid discussions, and countless tips and tricks from the ground, participants explored how quality improvement can be embedded within services to create meaningful and sustainable change.

What makes this programme special is not merely the knowledge shared, but the commitment demonstrated by participants to challenge existing practices, identify opportunities for improvement and champion quality within their respective organisations.
CHQR, IHSR is delighted to witness the emergence of new ideas, new collaborations and new service areas ready to embark on their quality journey. We look forward to seeing these teams translate learning into action and become catalysts for change within their services and communities.

To all services, specialties and states across the country — quality improvement is not a destination but a journey. Every champion started somewhere. The next success story could be yours.

Heartiest congratulations to the Bahagian Perkhidmatan Farmasi JKN Negeri Sembilan for successfully organising the Quality Assurance (QA) Project Workshop on 3 June!

The session brought together 39 pharmacists and pharmacy assistants, with technical guidance and training support provided by Dr Samsiah Awang from the Institute for Health Systems Research (IHSR).
Given the brief timeframe, this mini-workshop focused strictly on the vital steps of a QA project rather than the full cycle. Participants were taught how to identify and prioritise quality problems, building a strong foundation for future QA studies.
The practical session saw 10 working groups deep in discussion, brainstorming real-world operational challenges, deliberating on their impact, and utilising prioritisation techniques to select one core problem per group.
Interestingly, several overlapping challenges were identified across various facilities such as bedside dispensing issues in hospitals, TDM monitoring at KKs, and SPUB supply workflows that involve both hospitals and KKs. Instead of seeing these as roadblocks, we see them as an opportunity for multicentre and inter-facility QA studies, allowing the hospitals and KKs to solve systemic issues together!
Kudos to the organising team and all participants for their active participation and dedication to continuous quality improvement!
