PHILHEALTH REGISTRATION AND YAKAP REGISTRATION FOR STUDENTS

As part of the Municipality of Abuyog's continuing efforts to improve access to healthcare services and PhilHealth benefits, the Health Services Unit is facilitating the registration of students to ensure that they can avail themselves of the health programs and benefits available through PhilHealth and the Local Government Unit.

Step-by-Step Philhealth Check

1. Visit the portal. https://share.google/58slBdDsEGFRwv9YW
2. Complete all required personal information accurately. (A sample accomplished form is attached for your reference to help you correctly fill out the required fields.)
3. Review your information and submit for validation.
4. Wait for the validation check and take a screenshot. ( Note: for “transaction No:”) , (Note: Screenshot even if error”)

Note: Please bring a photocopy of your birth certificate or a valid government-issued ID, preferably your National ID, Brgy. Indigency (photocopy) and present a screenshot of your completed PhilHealth registration.

✅ Program Orientation by College for BEED, BSED, and BSCRIM students will be held on June 17.
