ISSUANCE OF EXHUMATION PERMIT
Requirements:
1. Death Certificate;
For non-dangerous communicable disease, permit may be granted such bodies have been buried for period of 3 years. For dangerous communicable disease, 5 years. Permit may be granted for shorter time in special case such as requested by court, police, NBI subject to the approval for the regional director concerned.
2. For special cases letter request by the Court, Police/ NBI (Medico-Legal Cases).
Where to Secure
1. Local Civil Register Office
STEPS
1. Proceed to Sanitation Office and present the requirements.
a. Check and review the requirements.
b. Release order of payment and instruct client to proceed to cashier.
2. Proceed to cashier and present the order of payment.
a. Receive payment and issue Official receipt.
3. Return to Sanitation Office and present Official receipt.
a. Check the Official receipt.
b. Release signed Exhumation Certificate
