The Online Appointment System is also open to accept appointments beginning July 06, 2026.

📢 PUBLIC ADVISORY

As of 6:00 PM, April 13, 2026

After several hours of service interruption, we are pleased to inform the public that the Online Health Card Appointment System is now back online.

Our Internet Service Provider began resolving a technical issue earlier today at around 4:00 AM and has since restored connectivity.

However, due to damage affecting their ICT infrastructure, intermittent instability may still be experienced while ongoing repairs are being completed.

We kindly ask for your patience and understanding as services continue to normalize.

We apologize for the inconvenience and thank you for your continued support.

Our Satellite Office at SM City Clark is fully operational to serve you for the issuance of Health IDs, pursuant to City Ordinance No. 326 series of 2012, and Presidential Decree No. 856.

The Satellite Office is located at Level 2, Cinema 5, SM City Clark, Angeles City

Steps:
1. REGISTER an account.
2. VERIFY your account sent thru your registered email.
3. CHOOSE your preferred DATE and TIME.
Our satellite office at SM City Clark is open to serve you from 10:00am until 6:00pm.
