📌 Basahin po nang mabuti ang mga dapat dalhin o requirements na kailangang ihanda at ipasa sa pagpapatala ng inyong anak.

📌 Sundin ang schedule ng pagpapatala ayon sa baitang ng inyong anak.

📌 Kumuha ng hard copy ng enrollment form/confirmation slip sa ating paaralan sagutan ang lahat ng detalye nang maayos at siguraduhing tama ang lahat ng impormasyon na inyong ilalagay.

📌 Magdala lamang ng sumusunod na dokumento sa pagpapatala:

✔️Para sa Kindergarten:
•Photocopy ng Birth Certificate
• Accomplished enrollment form

✔️Para sa Unang Baitang
•Photocopy ng Birth Certificate
•Orihinal na kopya ng Report Card
•Phil. ECCD Checklist
• Accomplished enrollment form

✔️Para sa ikalawa hanggang Ika-anim na Baitang
• Accomplished Confirmation slip

✔️Transferees/Balik-aral
•Photocopy ng Birth Certificate
•Orihinal na kopya ng Report Card o SF9
• Accomplished enrollment form

📌 Magdala rin po ng sariling ballpen na inyong gagamitin sa araw ng pagpapatala.
