ANNOUNCEMENT !!!
Sampaga Elementary School is now accepting Teacher Applicants for the school year 2022-2023.
All interested applicants must submit their pertinent documents on or before June 3, 2022.
Requirements:
1. Application letter addressed to the Schools Division Superintendent. Attention School Head
2. Omnibus certification of authenticity and veracity of all documents submitted, signed by the applicant and notarized.
3. CSC Form 212 (revised 2017)- Personal Data Sheet with latest passport size ID picture.
4. Copy of PRC ID and certified Copy of PRC Certification showing the name. LET rating and other information recorded in the PRC Office.
5. Photocopy of ratings obtained in the Licensure Examination for Teachers (LET)/PBET
6. Certified copy of Transcript of Records
7. Certification from the School of General Weighted Average (GWA)
8. Copies of service records or Certificate of Employment with inclusive dates for those with teaching experience.
9. Certificate of seminar/ trainings attended after the baccalaureate degree with inclusive dates. If not less than ten (10) days
10. Certificate of specialized Training. If any, authentication and veracity of all documents submitted: National Certificate issued by TESDA
11. NBI Clearance
12. Certified copy of the Voter's ID and/or any proof of residency as deemed acceptable by the School Screening Committee.
*The applicants are advised to bring with them the original documents and two sets of photocopy of said documents in correct order using white long folder.
