The applicant shall be required to provide the following data and sign a consent for its use for the program.

2. PhilHealth Identification Number
3. Date & Place of Birth
4. Address
5. Contact Number
6. Email Address

This healthcare program of Mayor Albee Benitez is open to all Bacolod residents.
