1. Photocopy of PSA Birth Certificate
2. Photocopy of Marriage Certificate (for married woman ONLY)
3. School credentials;
a. If JHS, SHS graduate – Form 137
b. If ALS Graduate – ALS Certificate
c. If College Undergraduate – Certificate of Grade from previous school
d. If College Graduate – TOR from previous school
4. Passport Size ID pictures, white background with complete name written
5. photocopy of Valid ID
