The Sangguniang Kabataan of Barangay An-Annam, Bantay, Ilocos Sur is pleased to announce the opening of nominations for the:

This recognition aims to honor and celebrate the outstanding young Bantayeños who have demonstrated excellence, dedication, and remarkable achievements in various fields.

ELIGIBILITY

Nominees must:

• Be a Bantayeño
• Be 10–19 years old
• Not have been a recipient in the same award category during the previous Pammadayaw

AGE CATEGORIES/

A. 10–14 years old
B. 15–19 years old

AWARD CATEGORIES:

• Academics
• Leadership
• Arts
• Journalism
• Sports

REQUIREMENTS:

All nominees are requested to prepare the following:

• Accomplished Nomination Form
• Summary of Accomplishments
• Certificates of Recognition/Achievement
• Awards, medals, and other proof of accomplishments
• Report Cards, if applicable
• Other relevant supporting documents
