Documentary Requirements:
a. Two Copies (2) of Application form for Doctor of Medicine Program (Note: Downloadable Application form is available on this link: https://tinyurl.com/Application-Form-Rev-3 )
b. Photocopies of Diploma and Transcript of Records of taken Bachelorโs degree program, supported by a certificate of class ranking and general weighted average.
c. Two (2) Letters of Recommendation from the former Dean or professor, and other government agencies (Mayor/Governor) addressed to the Dean of College of Medicine: Dr. Dolores V. Viliran
d. Certificate of Good Moral Character
e. Original and photocopy of NMAT result
f. If with parent/s with PWD, Certification from Attending Physician.
g. Original Copy of PSA Birth Certificate
h. If from Geographically Isolated and Disadvantaged Areas (GIDA), Calamity-Prone, Conflict Areas, and Indigenous Community, Certification from the Barangay or Certificate of Ancestral Domain (CADT), respectively
i. Solo Parent Certification, if necessary
j. Income Tax Return or Certificate of Indigency, if necessary
k. Health Clearance Report
-Complete immunization report (MMR, dT, Varicella, Hepatitis B)
-Chest x-ray result
-Blood typing result
-Drug Test result
-Urinalysis laboratory report
Procedure:
Step 1 : Submit the documentary requirements to Testing and Admission Office PB for initial review
Step 2 : Proceed to College of Medicine for evaluation
Step 3 : Secure a test schedule to TAO-PB
Step 4 : Take the admission test on the schedule indicated in the test permit
Step 5 : Proceed to College of Medicine for an interview
Step 6 : Receive admission results thru E-mail or a phone call from TAO-PB
