This activity aims to protect our children from measles and help promote a healthier and safer community. 💪👶

📢 NOTICE TO ALL UPLIFT BENEFICIARIES

Please prepare and submit the following requirements during your payout schedule August 11,2026 and August 12,2026:

1. Information Sheet
• 1 copy

2. General Intake Form
• 1 copy

3. Certificate of Eligibility
• 2 copies

4. Valid ID of the Beneficiary
• 3 copies ID must be printed front and back
• Provide 3 signatures at the lower portion of the bond paper

5. If the Beneficiary is Unavailable
• 3 copies of Authorization Letter with signature
• 3 copies of the beneficiary valid ID (front and back) with signature
• 3 copies of the authorized representative’s valid ID (front and back)

6. If the Beneficiary is Deceased
• 3 copies of the Death Certificate

📌 IMPORTANT:
If the beneficiary has no available valid ID, a Voter’s Certification will be accepted as an alternative.

Qualified students of Barangay San Pascual are now welcome to register for the ₱20,000 Educational Assistance Program under the Bawat Barangay Makikinabang Program.

📍 Registration: June 8–9, 2026
🏢 Venue: San Pascual Multi-Purpose Hall

📌 Only five (5) qualified college students will be selected as beneficiaries of this scholarship grant.

Qualifications:
✅ Incoming 4th Year College Student (SY 2026–2027)
✅ Bona fide resident of Barangay San Pascual
✅ Belongs to a vulnerable household or is at risk of discontinuing studies due to financial constraints
✅ Priority may be given to applicants from families with no college graduate
