Ang atong doktor sa CHO-Cabadbaran, si Dr. Kendrick Kerk Deguito, makahatag na og PhilHealth GAMOT prescriptions alang sa mga kwalipikadong pasyente nga rehistrado ug mipili sa CHO-Cabadbaran isip ilang PhilHealth YAKAP Provider. Pinaagi niini, mas sayon na ang pag-access sa mga tambal nga sakop sa programa alang sa atong mga pasyente.

Pinaagi sa PhilHealth YAKAP, ang mga pasyente mahimong magpakonsulta sa CHO-Cabadbaran ug, kung medikal nga gikinahanglan, makadawat og GAMOT Prescription alang sa mga tambal nga sakop sa programa. Apan sa pagkakaron, ang Pharmacy sa City Health Office dili pa accredited sa PhilHealth GAMOT Program. • Tabang gikan sa inyong napiling YAKAP Clinic

Lakang 3: Pilia ang CHO-Cabadbaran isip inyong YAKAP Provider.
Sa pagpili sa inyong YAKAP Clinic/Provider, pilia ang CHO-Cabadbaran.

Lakang 4: Bisitaha ang CHO-Cabadbaran alang sa konsultasyon.
Dad-a ang inyong PhilHealth details ug usa ka balido nga ID. Ang atong mga doktor motan-aw ug mo-assess sa inyong kahimtang sa panglawas.

Lakang 5: Kuhaa ang inyong GAMOT Prescription, kung gikinahanglan.

Lakang 6: I-claim ang inyong mga tambal sa usa ka accredited GAMOT provider o pharmacy.
