• National ID or PSA Birth Certificate (whichever is available)

Congratulations to Ms. Jessa Mae Baltazar, our dedicated Barkada Kontra Bisyo (BKB) President, for emerging as the CHAMPION in the recently concluded Municipal-Wide Poster Making Contest, one of the highlights of the 51st National Nutrition Month Celebration, sponsored by the Municipal Health Office of the Municipality of Cabugao.

Held today, July 29, 2026, at the Farmers Hall Multipurpose Hall, Cabugao, Ilocos Sur, this remarkable achievement reflects not only Jessa Mae's exceptional artistic talent and creativity but also her passion for promoting health, nutrition, and positive youth engagement through meaningful advocacy.
