Graduating students are NOT included po. Pasensya na po at tapos na ang payout nila.

๐ข CUTIES Submission Requirements
(For Incoming 3rd, 4th & 5th Year Students SY 2026โ2027)
Please prepare the following:

A. Application Form
Available sa mga barangay, leaders at offices of Dra. Bondoc in San Gabriel, Macabebe and Sta Cruz Poblacion San Luis
Original form po ang kailangan dahil naka numero po yan.

B. Photocopy of National ID (Claimant) and legal ID student
Very Important:
a) Sorry National ID lang ng CLAIMANT at wala na po iba. Rules ng DSWD po ito. Pasensyang pasensya na po. ๐๐๐
b) The address should be in the 4th District of Pampanga and should be the same address as the indigency.
c) Kung hindi students ang claimant: Photocopy of students ID (ok barangay ID, school ID, passport etc).
d) Kung walang National ID si student, maaring gawing claimant ang parents na may National ID
e) You may submit National ID from egov portal kung hindi dumating yung actual National
ID nila.

C. Proof of Voterโs Registration of claimant and student
Very Important
1) Must be registered in the 4th District of Pampanga
2) Tatanggapin namin Year 2020 -2026

D. Certificate of Registration (COR) (Portal copy is accepted)
โข 1st Semester, SY 2026โ2027
โข If not yet available: 2nd Semester, SY 2025โ2026

E. Certificate of Grades (COG) (Portal
copy ok)
โข 2nd Semester (SY 2025 - 2026)
โข If not available, 1st Semester 2025 to 2026 is accepted
โข No failing grades

F. Barangay Indigency Certificate (under CLAIMANTโS name)
Purpose must indicate: โFOOD ASSISTANCEโ
VERY IMPORTANT: National ID and Brgy. Indigency should have the same address.

G. Seeds / Binhi ๐ฑ
Claim your seeds/binhi and grow at least 10 plants at home as part of the program requirement
(Ipapamigay ito during submission)

If the claimant has received assistance from DSWD within the last 3 months, they are not qualified

โ No tita, tito, friend, or jowa
โ Someone else may submit your requirements for you
3๏ธโฃ Claimant must be available for pickup anytime
4๏ธโฃ Voters registration and National ID ok different address but dapat nasa 4th district of Pampanga yung address.
