Must be accompanied by Parent/Guardian with National ID
Present ANY of the following:
• Original Birth Certificate
• Barangay Certificate + Original Baptismal (for verification)
• Barangay Certificate (if no other document available)

🔄 UPDATING OF INFORMATION
1. Name / Birthday / S*x / Birthplace
• Original Birth Certificate
2. Marital Status (Single → Married)
• Original Marriage Certificate
3. Correction of Marital Status (Married → Single)
• CENOMAR
4. Address
• Original Barangay Certificate (Proof of Residence)
