Attached is the list of UPLIFT Program beneficiaries for Walang Gutom Program (WGP) beneficiaries.
📌 ISASABAY PO SA MISMONG FOOD REDEMPTION ANG PAY OUT NI UPLIFT.
📌 KUNG ANONG BARANGAY KA NAKA SCHEDULE, YUNG ORAS PO NA NAKA LAGAY SA SCHEDULE AND DAPAT PUNTAHAN. THE STAFF WILL NOT ENTERTAIN BENEFICIARY NA HINDI SUSUNOD SA ORAS NG SCHEDULE.
Requirements for beneficiary:
☑️ 1 photocopy of EBT card with 3 specimen signature
☑️ 1 photo copy of valid government issued id with 3 specimen signature
In cases where the EBT Card is LOST, SCRATCHED, DAMAGED or UNREADABLE OR WHERE THE CARD HAS NOT BEEN ISSUED by the concerned service provider:
☑️ 1 photo copy of valid government issued id with 3 specimen signature

In cases where the identified beneficiary is deceased (namatay), incapacitated, bedridden, suffering from serious illness, or other wise unable to personally claim the assistance, the next qualified household member appearing in the household record may be authorized to receive the assistance on behalf of the household, subject to validation and presentation of the following documents:
☑️ a proxy or authorized representative (who must be included in the household roster) shall present a valid government-issued identification verification, together with the beneficiary's EBT card. The representative must also submit the ORIGINAL authorization letter duly signed or thumbmarked by
beneficiary, indicating the following: (1) name
representative, (2) relationship to the beneficiary, and (3) reason for the beneficiary's absence during payout.
aside from securing the original authorization letter, the staff shall also secure the following documents:
☑️ A photocopy of the valid government-issued identification (ID) cards of both the beneficiary and the authorized representative, each bearing three (3) specimen signatures; and
☑️ A photocopy of the beneficiary's EBT card
b) For deceased (namatay) cases, the authorized representative shall be required to
present and submit the following:
☑️ A photocopy of valid government-issued
of the authorized
representative, bearing three (3) specimen signatures;
☑️A photocopy of the deceased beneficiary's EBT card; and
☑️ A photocopy of the beneficiary's death certificate. In case where the death certificate is not yet available, a certification issued by the barangay confirming the death of the beneficiary
c) No minor is allowed to participate in the UPLIFT payout, unless the concerned beneficiary is a minor who was onboarded under the WGP due to circumstances that prevents the parent/s, household head, or other.
PLEASE NOTE:
Bring the required documents to limit further delays.
❗️FOR THE AUTHORIZATION LETTER, IT MUST BE ORIGINAL (If si beneficiary ay nasa manila or abroad, malaki po ang tyansa na hindi makaka-claim dahil kailangan na original ang authorization letter) photocopy is not allowed.
